摘要
肿瘤转移是癌毒传舍过程。"留而不去,传舍于肠胃之外,募原之间。留著于脉,稽留而不去,息而成积"。癌毒传舍-转移成因。属内伤杂病传变,经络间传变即一经有病传至他经,或影响相连各经;经络脏腑传变乃经络至脏腑,邪气由浅入深;也有由脏腑至经脉;脏腑间传变更多。由腑及脏,其病较重,脏病难治;脏腑转移概率并不均等,肝、肺易成转移癌罹患器官,心、脾转移却罕有发生。癌毒传舍有规律,经络传舍、乘侮传舍、母子传舍等。藏象学说与现代医学解剖概念不尽相同,癌毒传舍不可盲目套用现代医学病名。痰毒流注-转移必要条件。痰的流动使癌毒随痰播散周身,痰的留着、黏滞使癌毒易在某些脏器组织中形成转移灶。痰毒流注之毒为内生之毒,乃阴阳失和、气血运行不畅及脏腑功能失调导致,多由诸邪蓄积,胶结壅滞成毒。流注于肝成肝积,流注于肺成肺积,流注于经络成瘰疬。正气虚弱-转移促进因素。恶性肿瘤全身属虚、局部属实,始终表现正气为癌毒耗散,特别是晚期转移者,随病程进展,气血津液耗损,脏腑失养,正气虚弱会愈加明显;癌毒稽留而不去,治疗攻伐太过,致正虚邪陷,促进癌毒扩散、传舍至机体局部成为新病灶。虚证在恶性肿瘤病程,尤其中晚期是极为重要病理因素。未来期待引入现代科研先进方法,探讨辨证分型与肿瘤生物学行为及转移诸因素之间相关性,尤其是与肿瘤转移密切相关的基因、黏附分子、血管生长因子等,筛选出有分子生物学证据的与肿瘤转移高度相关的证候,从而丰富和发展肿瘤转移的中医病机学说。
