摘要
目的探讨鼻内镜对顽固性鼻出血确切出血部位的诊断意义,并观察在鼻内镜下用自制电凝头烧灼法治疗顽固性鼻出血的效果。方法对48例经前鼻孔或后鼻孔填塞后再次出血的患者行鼻腔表面麻醉后,在鼻内镜下吸除鼻腔内血凝块,仔细寻找到出血部位或出血点后,用自制电凝头点击烧灼出血点至血止。结果术后随访6个月,48例顽固性鼻出血患者中一次性止血成功者45例,二次成功者3例,止血后平均住院时间为2 d。结论鼻内镜检查有助于准确寻找到出血部位,自制电凝头烧灼止血法能明显减轻病人痛苦、提高治愈率和缩短住院时间,是治疗顽固性鼻出血的有效、安全、简洁的方法,值得临床推广使用。
Objective To discuss the role of nasal endoscope on determining bleeding-site in obstinate epistaxis,and observe the effect of electriccoagulation with self-made probe on curing obstinate epistaxis.Methods Total 48 re-bleeding patients after anterior nasal packing or posterior nasal packing underwent nasal topical anesthesia,the nasal blood clots were removed guided by nasal endoscope,the bleeding sites or points were confirmed and cured by electriccoagulation with self-made probe.Results All patients were followed up for average 6 months after operation,the bleeding in 45 cases were stopped by one time,3 cases by the second time.The average hospital stay after operation is 2 days.Conclusion The nasal endoscope is helpful for finding bleedingsite exactly.The cauterization hemostasis by using of self-made probe can relieve the patients’suffering,improve the cure rate,and shorten the average hospital stay.It is an effective,safe,handy method on curing obstinate epistaxis.
