摘要
目的:研究鼻内镜下双极电凝与微波治疗老年难治性鼻出血的临床效果。方法:选取2014年3月-2016年6月笔者所在医院收治的90例老年难治性鼻出血患者作为研究对象。按照随机数字表法将其分为三组,每组30例。对照组予以常规鼻腔填塞治疗,电凝组予以鼻内镜下双极电凝治疗,微波组予以鼻内镜下双极微波治疗。比较三组临床指标及治疗效果。结果:电凝组及微波组术中出血量均明显少于对照组,且电凝组少于微波组,差异有统计学意义(P<0.05)。电凝组与微波组鼻腔通气恢复时间、鼻腔黏膜恢复时间均明显早于对照组,且电凝组均早于微波组,差异有统计学意义(P<0.05)。电凝组及微波组治疗总有效率均高于对照组,且电凝组高于微波组,差异有统计学意义(P<0.05)。结论:鼻内镜下双极电凝治疗老年难治性鼻出血有着非常好的临床效果,应为治疗时候的首选。
Objective:To study the clinical effect of bipolar electrocoagulation and microwave under nasal endoscope in the treatment of intractable epistaxis in the elderly.Method:A total of 90 elderly patients with intractable epistaxis admitted in our hospital from March 2014 to June 2016 were selected as the research objects.According to the method of random number table,they were divided into three groups,with 30 cases in each group.The control group received conventional nasal packing therapy,the electrocoagulation group received bipolar electrocoagulation under nasal endoscope therapy,and the microwave group received bipolar microwave under nasal endoscope therapy.The clinical indicators and therapeutic effects of the three groups were compared.Result:The amount of intraoperative blood loss in both electrocoagulation group and microwave group was significantly less than that in the control group,and the electrocoagulation group was less than that in the microwave group,the differences were statistically significant(P<0.05).The recovery time of nasal ventilation and nasal mucosa of the electrocoagulation group and the microwave group were significantly earlier than that of the control group,and the electrocoagulation group was significantly earlier than that of the microwave group,the differences were statistically significant(P<0.05).The total effective rate of treatment in both electrocoagulation group and microwave group was higher than that in the control group,and the electrocoagulation group was higher than that in the microwave group,the differences were statistically significant(P<0.05).Conclusion:Bipolar electrocoagulation under nasal endoscope for the treatment of intractable epistaxis in the elderly has a very good clinical effect and should be the first choice for treatment.
