RBP4与hsCRP对妊娠糖尿病诊断价值的研究 被引量:8

摘要 目的探索视黄醇结合蛋白4(RBP4)与高敏C反应蛋白(hsCRP)联合检测对妊娠糖尿病诊断的临床意义。方法对2012年1~12月在本院就诊的孕期170~230 d无糖尿病病史孕妇进行50 g葡萄糖筛查(GCT),异常时再行75 g的口服葡萄糖耐量试验(OGTT)检查。糖尿病者纳入GDM组,无糖尿病者纳入CON组。采集两组患者血清,利用免疫投射比浊法检测hsCRP水平,ELISA方法检测RBP4水平。分析两种蛋白在妊娠糖尿病(GDM)孕妇血清中的中位数及最佳临界值,并利用临界值观察二者联合检测诊断GDM的敏感性和特异性,同时与临床确诊结果进行一致性分析,检验联合诊断有效性。结果 GDM组BRP4和hsCRP的中位数分别为52.3 ng/ml和4.51 mg/L,显著高于CON组的中位数分别为43.6 ng/ml和1.49 mg/L(P<0.01)。BRP4和hsCRP联合诊断GDM的敏感性和特异性分别为91.4%和93.6%,且与临床确诊结果具有良好的一致性。结论 BRP4和hsCRP的联合检测可作为GDM的早期诊断方法。 Objective To explore the clinical value of retinol binding protein-4(RBP4) combined with high-sensitive Creactive protein(hsCRP) in the diagnosis of gestational diabetes mellitus(GDM).Methods From January to December in 2012,the pregnant women in 170-230 d of pregnancy without the history of diabetes mellitus(DM) received 50 g glucose challenge test(GCT).Those having abnormal results again received 75 g oral glucose tolerance test(OGTT).Those with DM were divided into GDM group,while those without DM were divided into CON group.The serum samples of both groups were obtained.Immunoturbidimetriy was used to detect the hsCRP level,and ELISA assay was used to detect the RBP4 level.The medium value of both proteins and the best critical value in the serum of the pregnant women with GDM were obtained by statistical analysis.The critical value was used to observe the sensitivity and specificity of the integrated detection method,and the concordance with the diagnostic results was analyzed in order to identify the efficiency of the integrated detection.Results The medium values of BRP4 and hsCRP in the GDM group were 52.3 ng / ml and 4.51 mg / L,respectively,which were significantly higher than those in the CON group(43.6 ng / ml and 1.49 mg / L,P &lt; 0.01).The sensitivity and specificity of the integrated diagnosis with BRP4 and hsCRP on GDM were 91% and 93%,respectively,which had a good concordance with the clinical diagnostic results.Conclusion The integrated detection with RBP4 and hsCRP can be used as an early diagnostic method for GDM.
