摘要
目的探讨初诊2型糖尿病患者血清视黄醇结合蛋白(RBP4)与血脂及胱抑素c的关系。方法选取研究对象152例,其中新诊断2型糖尿病伴血脂异常组39例,新诊断2型糖尿病血脂正常组39例;无2型糖尿病血脂异常组36例,无2型糖尿病血脂正常组38例。测定受试者的体重指数(BMI)、腰臀比、血压、机体脂肪百分比(F%),检测4组空腹血清RBP4、血脂和胱抑素C水平,对血清RBP4、血脂及胱抑素c进行单因素相关分析,然后进行多元逐步回归分析。结果糖尿病血脂正常组的RBP4水平高于无糖尿病血脂正常组(P〈0.05),糖尿病血脂异常组的RBP4水平高于无糖尿病血脂异常组(P〈0.05),单因素相关分析显示,RBP4与总胆固醇(TC)、甘油三酯(TG)、空腹血糖(FPG)、载脂蛋白(Apo)A、ApoB、低密度脂蛋白胆固醇(LDL—C)、BMI和F%呈正相关(均P〈0.01);RBP4与高密度脂蛋白胆固醇(HDL—C)、ApoA、ApoA/ApoB比值呈负相关(均P〈0.05)。多元逐步回归分析显示,ApoA、ApoB、ApoA/ApoB比值、BMI和TC是RBP4的影响因素,RBP4与ApoA、ApoA/ApoB比值呈负相关,RBP4与ApoB、BMI和TC呈正相关(均P〈0.05)。RBP4与胱抑素C无相关性。结论在初诊2型糖尿病患者中,RBP4与ApoA、ApoB相关,而与胱抑素C无关。
Objective To explore the relationship of serum retinol-binding protein 4 (RBP4) with cystatin-C and blood lipid in patients with newly-diagnosed type 2 diabetes mellitus (T2DM). Methods A total of 152 subjects were investigated, including 78 patients with newly-diagnosed T2DM (39 with dyslipidemia and 39 with normal lipidemia) and 74 control subjects (36 with dyslipidemia and 38 with normal lipidemia ). The anthropometrical parameters such as body mass index ( BMI ) , percentage of body fat ( F% ) , blood pressure, and waist hip ratio (WHR) were evaluated. Fasting serum RBP4, blood lipid, and cystatin-C levels were determined. Univariate analysis was performed for RBP4, blood lipid, and cystatin-C, and then multiple regression analysis was made. Results The level of serum RBP4 was significantly higher in T2DM group with normal lipidemia than that in control group( P〈0.05 ). The level of serum RBP4 was significantly higher in T2DM group with dyslipidemia than that in control group with dyslipidemia ( P〈0.05 ). Univariate analysis showed that RBP4 was positively correlated with total cholesterol (TC) , triglyceride (TG) , fasting plasma glucose ( FPG ) , apolipoprotein ( Apo ) A, ApoB, low density lipoprotein-cholesterol( LDL-C ), BMI, and F%. RBP4 was negatively correlated with high-density lipoprotein cholesterol, ApoA, and ApoA/ApoB ratio. Multiple regression analysis showed that ApoA, ApoB, ApoA/ApoB, BMI, and TC were the influential factors of RBP4. RPB4 was negatively correlated with ApoA and ApoA/ApoB ratio, and positively correlated with ApoB, BMI, and TC. RBP4 showed no correlation with cystatin-C. Conclusion RPB4 was correlated with ApoA and ApoB, but not with cystatin-C.
