摘要
目的探讨青海地区藏族人群CYP2E1基因多态性和环境因素的交互作用,以及与原发性肝癌的关系。方法病例对照研究。将青海地区藏族原发性肝癌患者(病例组)、同期健康藏族志愿者(对照组),应用PCR、DHPLC法检测两组人群CYP2E1基因多态性位点。以非条件logistic同归模型分析比较CYP2E1基因型在两组分布频率的差异,以及基因多态性和环境因素的交互作用。结果青海地区藏族人群中CYP2E1基因5'-端位点3种基因型c1/c1、c1/c2、c2/c2在病例组和对照组中分布频率分别为84.2%、15.8%、0.0%和86.6%、13.4%、0.0%,两组差异无统计学意义(P>0.05);携带野生纯合子c1/c1基因型个体罹患PHC的风险是c1/c2基因型的5.33倍(95%CI:0.413~15.517),但差异无统计学意义(P>0.05);经logistic逐步回归分析结果显示,吸烟OR 3.808(95%CI:1.742~8.321);家族肝炎病史OR14.195(95%CI:4.304~46.809);家族肝癌史OR5.851(95%CI:1.972~17.360)。结论 CYP2E1基因与青海地区藏族肝癌发生可能无密切关系;藏族人群CYP2E1基因多态性与吸烟、直系亲属肝癌家族史、直系亲属肝炎病史在肝细胞癌发生中存在交互作用,能增加罹患原发性肝癌的风险。
Objective To explore the cytochrome P4502E1 (CYP2E1)gene polymorphism with environmental factors interaction and relationship with primary liver cancer(PHC) of Tibetan populations in Qinghai. Method Ap- plied of Polymerase Chain Reaction(PCR) and denaturing high performance liquid chromatography( DHPLC ) to de- tect the Tibetan patients with PHC and CYP2E1 gene polymorphism of healthy Tibetan population. Non - conditional logistic regression model was used to compare the CYP2E1 genotype differences with the frequency distribution,the gene polymorphism and the environmental factors in the two groups. Results The CYP2E1 gene 5' - end of the locus genotypes and the CYP2E1 cl/cl cl/c2 of c2/c2 distribution between the case and control groups was not statisti-cally significant(P 〉 0.05 ), the homozygous wild cl/cl genotype in the CYP2E1 genetic 5' - end sites, Carrying cl/c2 or c2/c2 genotype of CYP2E1 gene 5' - end sites suffer no significant increase in risk of hepatocellular carci- noma(P 〉 0.05 ), Stepwise logistic regression analysis results show that the interaction between CYP2E1 gene and smoking, mmediate family history of PHC family history and the immediate family members of hepatitis in PHC, The interaction OR values were 4.115,5. 418,. 593, espectively. Conclusions CYP2E1 gene polymorphism and smok- ing, he immediate family hepatoma and immediate family history of hepatitis show interaction with PHC and it may increase the risk of PHC.
