摘要
目的分析2011年鲍氏不动杆菌医院感染分布及耐药性,为临床治疗和医院感染控制提供依据。方法对临床送检标本进行分离、培养,采用VITEK-32细菌鉴定仪对鲍氏不动杆菌进行鉴定,使用纸片扩散法测定菌株对抗菌药物的敏感性,所有数据应用WHONET5.5及SPSS13.0软件进行统计分析。结果分离鲍氏不动杆菌188株,以痰中检出率最高,占79.3%;科室分布以ICU、呼吸科为主,分别占39.9%和12.2%;该菌对抗菌药物总耐药率以米诺环素最低,为14.9%,对其他常用抗菌药物的耐药率>50.0%,而ICU除米诺环素外,对各类抗菌药物耐药率>80.0%。结论鲍氏不动杆菌耐药严重,集中分布于ICU,重视对鲍氏不动杆菌的耐药监测,采取有效措施避免和减少多药耐药的鲍氏不动杆菌传播。
OBJECTIVE To analyze the nosocomial infection distribution and antimicrobial resistance of isolated Acinetobacter baumannii in 2011so as to provide reference for clinical treatment and nosocomial infection control.METHODS The isolation of the bacterium from clinical specimens was conducted and A.baumannii species identification was performed according to the microorganical method of VITEK32 system.Antimicrobial susceptibility testing was performed by disk diffusion method.All the data were analyzed by WHONET5.5and SPSS13.0software.RESULTS A total of 188strains of A.baumannii were isolated,which mainly came from the sputum samples of patients(79.3%).In the distribution of hospital departments,the ICU ranked the first place(39.9%),the respiratory department the second(12.2%).The antibiotics susceptibility testing showed that the lowest resistance rate of A.baumannii to 16kinds of antimicrobial agents was 14.9% of minocycline,and the resistance rate to other antimicrobial agents was more than 50.0%.The A.baumannii isolated from ICU patients were more resistant to the commonly used antimicrobial agents than those isolated from non-ICU patients(P 0.01),the resistance rate to antimicrobial agents was more than 80.0% with the exception of minocycline in ICU.CONCLUSIONThe A.baumannii strains are highly resistant to antibiotics.These strains are mainly isolated from ICU.We should focus on the monitoring of the drug resistance and take the effective measures to prevent and avoid the spread of the multidrug-resistant A.buamannii.
