颅内动脉瘤栓塞致密度与复发的血流动力学分析 被引量:13

摘要 目的探讨颅内动脉瘤栓塞过程中,瘤腔内血流动力学变化规律,为防止颅内动脉瘤栓塞后复发提供血流动力学理论参考。方法应用图像测量软件分析15例颅内动脉瘤DSA图像中选定区域像素随时间变化的密度值,绘成时间-密度曲线,并根据时间密度曲线进一步分析动脉瘤栓塞前后血流速度变化。结果15例颅内动脉瘤中栓塞密度达到Ⅰ°的9例患者无1例复发;2例Ⅱ°患者有1例复发;4例Ⅲ°患者全部复发。5例复发患者动脉瘤残留区内测得的相对流入系数ACKa/PKa值均接近或高于栓塞前。结论瘤腔内血流速度与动脉瘤栓塞后残腔大小呈负相关,即不完全栓塞的动脉瘤腔内血流在治疗后显著加速,这或许能够解释不完全栓塞动脉瘤复发率高的原因。
