摘要
目的探讨经颈内静脉置管治疗下肢深静脉血栓(DVT)的临床应用价值。方法回顾性分析53例下肢深静脉血栓患者,其中38例行颈内静脉置管后采用抗凝及经溶栓导管脉冲式灌注尿激酶综合治疗下肢静脉血栓(包含11例行下腔静脉滤器置入术),另15例行患侧大隐静脉入路置管溶栓。术后3、6、12个月行彩超复查评价静脉通畅程度。结果经颈内静脉置管溶栓组患者术后临床症状均明显缓解或消失,彩超提示下肢深静脉血流通畅。随访1年,无出血、肺动脉栓塞等严重并发症。综合有效率达100%(38/38),高于大隐静脉入路置管溶栓组的86.7%(13/15),在α=0.1的水平具有统计学显著性(P=0.076)。结论经颈内静脉置管治疗下肢深静脉血栓是一种安全有效的治疗方法。
Objective To investigate the clinical application value of internal jugular vein catheter treatment of lower limb deep vein thrombosis （DVT）. Methods Fifty-three cases patients with DVT were selected, 38 of them performed anticoagulation and thrombolysis catheter pulse infusion of urokinase after internal jugular vein indwelling catheter （group A, 11 cases placed inferior vena cava filter included,）. 15 cases were performed ipsilateral great saphenous vein catheter thrombolysis （group B ）. Postoperatively, the venous patency was evaluated after 3,6,12 month by color doppler ultrasound. Results The clinical symptoms of all patients in group A were significantly ease and disappear af- ter operation. Ultrasonography indicated lower extremity deep venous patency unobstructed. The results of following up patients for 1 year suggested that recent curative effect was satisfactory and no complications of bleeding and pul- monary embolism. Comprehensive effective rate was 100%（38/38）, was higher than 86.7%（13/15） of group B, was sig- nificantly at the level of α=0.1 （P = 0.076）. Conclusion It is a safe and effective method to thearp DVT in internal jugular vein indwelling catheter
