摘要
采用无创性光电容积描记(PPG)和空气容积描记(PCR)检测44例(57条)下肢浅静脉曲张患者和12例(24条)正常人。结果表明:PPG检测静脉瓣膜功能敏感性为96.5%,特异性为100%,诊断正确率为97.5%,对深静脉3~4级返流PPG的符合率为86.7%。PCR检测静脉通畅度,PCR比值和曲线符合率分别是90.9%和86.4%,且深静脉3~4级返流组静脉回复时间(VRT),节段性静脉容积(SVC)和静脉最大回流量(MVO)与1~2级返流组和正常对照组比较,差异有显著性意义(P<0.05),术后VRT较术前明显增高(P<0.001)。结果提示:PPG结合PCR检测对诊断静脉疾病,评价血流动力学和疗效,是一种客观有效的无创性检测方法。
By employing non-invasive photoplethysmography (PPG) and pressure cuff recording (PCR), 44 patients (57 limbs) with varicosity and 12 normal subjects (24 limbs) were examined. The results were compared with those of venography and ultrasonic Doppler examination. The results indicated that the sensitivity of PPG examination of venous valve competence was 96.5%, the specificity was 100%, and rate of correct diagnosis was 97.5%. PPG accuracy for detecting the deep vein 3-4 stage reflux was 86.7%. PCR ratio and graph conformity rate were 90.9% and 86.4% in testing venous patency respectively. Moreover, there were significant differences in venous refill time (VRT), segment venous capacitance (SVC) and maximum venous outflow (MVO) of deep vein 3-4 stage reflux between those patients with 1-2 stage reflux and normal subjects (P<0.05). VRT obviously increased after operation P<0.001). These suggested that PPG combined with PCR test is an objective, effective and non-invasive technique in the diagnosis of venous diseases and evaluation of hemodynamics.
