摘要
目的金黄色葡萄球菌肠毒素B(staphylococcal enterotoxin B,SEB)作为超抗原(superantigen)目前被认为可能是导致鼻窦炎发生、发展并向慢性转化的主要因素之一。文中通过向兔上颌窦腔注入不同浓度SEB,明确其能否成功诱导急性上颌窦炎。方法 45只兔随机分成3组,每组15只。每天分别向兔右上颌窦腔注入低浓度SEB(0.3 ng/ml,A组)、中等浓度SEB(3 ng/ml,B组)及高浓度SEB(30ng/ml,C组)各2ml,3组兔左上颌窦腔每天注入2ml等渗盐水作为对照侧。3d、7d及14 d后每组随机选取5只兔处死,取双上颌窦分泌物行普通细菌培养及鼻窦黏膜标本光镜下观察。结果 3 d后,3组兔右上颌窦分泌物普通细菌培养结果均为阴性;7d及14d后,A组右上颌窦分泌物普通细菌培养结果为阴性,B、C组右上颌窦分泌物普通细菌培养阳性率均为100%。光镜下黏膜病理半定量结果示:B、C组与A组比较,3 d后炎性细胞浸润差异有统计学意义(P<0.05),7d及14d后炎性细胞浸润及上皮溃疡形成差异均有统计学意义(P<0.05);C组与B组比较,7d及14d后上皮溃疡形成差异均有统计学意义(P<0.05)。3组兔左上颌窦细菌培养结果均为阴性,黏膜形态学未见明显改变。结论持续灌注中高浓度SEB 7 d可成功诱导急性鼻窦炎,且随着SEB浓度增加鼻窦黏膜形态学改变更加显著。
ObjectiveRecent studies suggest that staphylococcal enterotoxin B (SEB) is capable of acting as superantigen and may play an important role in the occurrence and progression of sinusitis as well as in the development of acute sinusitis into chronic one. The aim of this study was to investigate whether the injection of different concentrations of SEB into the maxillary sinus can induce experimental acute maxillary sinusitis in rabbits.MethodsForty-five rabbits were equally randomized to groups A (low-dose), B (medium-dose) and C (high-dose) to receive daily instillation of SEB into the right maxillary sinus at 0.3, 3 and 30 ng/ml, respectively, and all were instilled with a daily dose of normal saline into the left maxillary sinus. At 3, 7 and 14 days, 5 animals from each group were killed for routine culture of the secretions from bilateral maxillary sinuses and observation of the sinus mucous membrane under the light microscope.Results At 3 days, no bacteria were isolated from the secretions of the right maxillary sinus in the three groups, and at 7 and 14 days, it was the same case with group A, but the positive rate of bacterial culture was 100% in both B and C. Light microscopy revealed a milder infiltration of inflammatory cells in group A than in B and C at 3 days (P〈0.05). At 7 and 14 days, both epithelial ulceration and inflammatory cell infiltration were less pronounced in A than in B and C (P〈0.05), the former also less pronounced in B than in C (P〈0.05). There were no bacteria isolated from the secretions, nor any histopathological changes in the left maxillary sinus of the three groups of rabbits.ConclusionDaily instillation of medium- or high-dose SEB for 7 days can induce acute sinusitis, and histopathological changes in the sinus mucous membrane become more obvious with increased dose of SEB.
