摘要
目的探讨呼吸道清除系统(Vest系统)预防老年患者术后肺部感染的作用,为临床预防提供依据。方法选取医院老年腹部手术患者80例,随机分为试验组和对照组各40例,试验组实施Vest系统排痰法,对照组采用人工叩击排痰法,对试验组和对照组患者排痰量、心率、血压等血流动力学、伤口引流量、患者疼痛以及体位舒适度等数据进行比较。结果试验组患者在第1～4天的排痰量分别为(24.35±2.14)、(36.08±2.44)、(21.23±1.99)、(32.38±2.25)ml/d,对照组分别为(14.20±1.59)、(17.31±1.67)、(15.05±2.19)、(17.21±2.46)ml/d,两组差异均有统计学意义(P<0.05);试验组发生切口疼痛患者7例、VAS评分(4.2±0.3)分,对照组22例、(7.1±0.4)分,差异均有统计学意义(P<0.05);试验组患者体位舒适率90.0%、配合率92.5%,对照组为47.5%、57.5%,差异均有统计学意义(P<0.05);两组在血流动力学方面差异无统计学意义;试验组术后发生肺部感染显著少于对照组,差异有统计学意义。结论 Vest系统对老年手术患者具有良好的排痰效果,可有效预防术后肺部感染的发生。
OBJECTIVE To investigate the effect of airway clearance system （Vest TM system） on the pre,Tention of postoperative pulmonary infections in the elderly patients so as to guide the clinical prevention. METHODS A total of 80 elderly patients undergoing abdominal operation were selected and randomly divided into the observation group and the control group with each group of 40 cases. The observation group was treated with Vest TM system. The control group was treated with artificial knocking expectoration. The sputum volume, hemodynamie of heart rate and blood pressure, drainage fluid volume, wound pain, and position comfort level in two groups were compared. RESULTS The sputum volume on the 1st, 2nd, 3rd, and 4th day were respectively （24.35±2.14 ） ml/d （36.08±2.44）ml/d, （21.23±1.99） ml/d , and （32.38±2.25） ml/d in the observation group and were respectively （14.20±1.59）ml/d, （17. 31±1.67） ml/d, （15.05±2.19）ml/d, and （17. 21±2.46） ml/d in the control group, the difference between the two groups was statistically significant （P〈0.05） ; there were 7 cases with cut pain in the observation group and 22 cases in the control group, the VAS score was （4.2±0.3） points in the observation group and （7.1 ± 0.4） points in the control group , the difference was statistically significant （P〈 0.05） ; the position comfort rate was 90.0% in the observation group and 47.5% in the control group, the fitting degree was 92.5% in the observation group and 57.5% in the control group, the difference was statistically signif- icant （P〈0.05） ; the difference in the hemodynamics between the two groups was not statistically significant the incidence of the postoperative pulmonary infections of the observation group was significantly lower than that of the control group , the difference was statistically significant （P〈0. 05）. CONCLUSION Vest TM system has favorable effect on the expectoration of the elderly patients and can effectively prevent the postoperative pulm
