摘要
目的探讨定时吸痰管经鼻刺激咽喉部引发反射性咳嗽,促进气道深部排痰的方法对老年肺部感染患者的无创排痰的疗效。方法选择不能完成主动咳嗽排痰动作的老年肺部感染患者46例,随机分为A组和B组,每组各23例;A组采用吸痰管定时经鼻刺激法,B组采用传统常规吸痰法,比较两组治疗10d后肺部体征改善情况、抗菌药物使用时间、总有效率。结果治疗10d内呼吸音明显改善A组19例、B组13例,A组明显多于B组(χ2=5.2541,P=0.0219),胸部CT结果显示,治疗第10天炎症吸收>80%A组有19例,B组有13例,A组显著多于B组(χ2=4.2125,P=0.0401),治疗总有效率A组为95.65%、B组为73.91%,结果A组优于B组,差异有统计学意义(χ2=4.2125,P<0.05)。结论采取吸痰管定时经鼻刺激咽喉部引发反射性咳嗽,促进气道深部排痰,提高肺部感染的疗效。
OBJECTIVE To explore the efficacy of noninvasive expectoration for the elderly patients with pulmonary infections by timing suction tube stimulation through nose to throat to trigger a reflex reflective cough and then to promote the deep airway expectoration. METHODS A total of 46 elderly patients with pulmonary infections who could not achieve active cough row of phlegm action were selected and randomly divided into the group A and the group B with 23 cases in each group. The patients in the group A were treated by using the timing sputum suction tub stimulation method , while the patients in the group B received the conventional suction method. After the treatment for 10 days ,the improvement in pulmonary signs, time of use of antibiotics, and the total effective rate between the two groups were compared. RESULTS There were 19 cases of patients in the group A with the breath sound significantly improved within 10 days of the treatment , significantly Iflore than the 13 cases in the group B （χ2 =5. 2541, P=0. 0219）; the chest CT result indicated that on the tenth day of the treatment there were 19 cases of patients in the group A with the inflammation absorption more than 80%, significantly more than the 13 cases in the group B （χ2 =4. 2125, P=0. 0401）, and the total effective rate of the group A was 95. 65% , significantly higher than 73.91% of the group B , the difference was statistically significant （χ2 = 4. 2125, P〈0.05）. CONCLUSION The timing sputum suction tube stimulation through nose to throat can trigger the reflex cough which may promote the airway deep expectoration and improve the efficacy in treatment of pulmonary infections.
