摘要
目的探讨浆细胞性乳腺炎（plasma cell mastitis,PCM）的诊断和治疗方法,提高对该病的诊治水平.方法回顾性分析60例术前或术后诊断为浆细胞性乳腺炎患者的临床资料,总结其诊断与治疗方法.结果60例患者中,术前诊断为浆细胞乳腺炎28例,占46.7%,其中22例术后确诊为浆细胞乳腺炎,另6例术前诊断为浆细胞乳腺炎,术后快速病理提示乳腺癌2例,乳腺小叶增生2例,乳腺纤维瘤2例.其余32例中术前误诊为乳腺其它疾病,术后均确诊为浆细胞乳腺炎.结论该病临床表现较复杂,易误诊为乳腺癌、乳腺纤维瘤、炎性包块等,需采用快速冰冻切片法及常规病理切片确诊,手术治疗是该病的最佳方案.
Objective To explore the diagnosis of plasma cell mastitis and treatment methods,improve the level of realization of the disease.Methods A retrospective analysis of 60 cases of plasma cell mastitis with clinical data before or after surgery,summarize the methods of diagnosis and treatment.Result Among 60 cases,there are 28 cases diagnosed plasma cell mastitis before operation with the percentage of 46.7%.22 of the 28 cases make a definite diagnosis as plasma cell mastitis after operation,the left diagnosed as other diseases such as 2 cases of breast cancer,2 cases of fibroadenoma of breast and 2 cases of hyperplasia of mammary gland flocculus after operation.32 of the 60 cases diagnose plasma cell mastitis after operation,that misdiagnose other breast diseases before operation.Conclusion The clinical feature of this disease is sophisticated,with misdiagnoses like breast cancer,fibroadenoma of breast,and inflammatory lump.The only way to make a definite diagnosis is the rapid frozen section and routine pathology.Operation is the optimal treatment program.
