摘要
目的探索社区卫生服务中心开展心房颤动(房颤)患者综合治疗管理中健康教育的有效方案,以提升患者对疾病管理的依从性。方法建立房颤管理窗口,筛查在本中心就诊的房颤患者纳入管理,评估、调查、制定个体化综合健康教育方案,进行实施和随访。结果被纳入房颤管理窗口有效管理房颤患者105例,失访3例,管理率97.2%,平均管理随访11.2个月,CHADS2评分≥2分者79例(75.2%),服用华法林12例(15.2%),合理转诊11例。实施发放健康教育手册、公开房颤管理窗口服务电话、房颤管理窗口强化宣教、制定患者自我管理方案等个体化综合健康教育方案,结合其它综合治疗管理方案,管理前后患者对房颤相关知识认知率、服用华法林比率、治疗依从性和满意度提高。结论社区建立房颤管理窗口,实施有效的个体化综合健康教育方案,可提高患者对综合治疗管理的依从性和满意度。
Objective Explore the community health service centers carrying out effective method of health education in the comprehensive treatment and management of patients with atrial fibrillation （AF）, to enhance patient adherence to disease management. Methods Established atrial fibrillation management window to screen patients with atrial fibrillation to be included in the center for treatment management, assessments, surveys, develop individu- alized comprehensive health education program, and implement it and follow - up. Results To have 105 cases of pa- tients with atrial fibrillation come into the effective management of the atrial fibrillation management window, loss of follow three eases and the management rate is 97.2%. average management follow - up 11.2 months, 79 cases of CHADS2 score ≥2 points （75.2%）, 12（ 15.2% ） patients taking warfarin, and reasonable referral of 11 eases. The implementation of patient self - management programs, included： health education manual, open atrial fibrillation management window service telephone, atrial fibrillation management window intensive education, strengthen the mis- sion to develop individualized health education programs. In combination with other therapy management program, comparing with the previous management, the patients taking warfarin have increased; have more recognition of atrial fibrillation -related knowledge, improved treatment adherence and satisfaction. Conclusion Establish community at- rial fibrillation management window and implement effective individualized comprehensive health education program can improve patient compliance and satisfaction of comprehensive treatment management.
