摘要
目的:观察雌激素的不同起始时间对子宫内膜厚度的影响及对冻融胚胎复苏移植FET(frozen-thawed embryotransfer)结局的影响。方法:采用前瞻性随机的方法,将雌激素准备子宫内膜方案患者(共702周期)分为A组(D1用药)和B组(D5用药),比较两组应用黄体酮转化子宫内膜日(简称P日)子宫内膜厚度、P日前雌激素的总剂量及持续时间、妊娠率及流产率等指标,同时评估子宫内膜厚度与FET结局的关系。结果:A、B两组P日的子宫内膜厚度分别为(8.8±1.23)mm和(9.14±1.40)mm,P日前雌激素的总剂量及持续时间A组为(83.97±37.5)mg和(15.83±3.01)天,B组为(67.42±26.19)mg和(11.40±2.81)天,两组比较差异均有统计学意义(P<0.05);妊娠率、流产率A、B两组分别为(45.1%VS47.1%)、(6.1%VS6.7%),两组差异均无统计学意义(P>0.05)。结论:雌激素不同起始时间准备子宫内膜的方案对FET结局无明显影响;B组方案获得的内膜厚度更为理想,更能模拟生理,更有利于子宫内膜的生长。
Objective: To observe the effect of artificial endometrial stimulation protocols with different starting time of estrogen on outcome of FET cycles and endometrial thickness. Methods : A prospective study was conducted on 702 cycles of FET for artificial endome- trial stimulation protocols with estrogen, treatment began on cycle day 1 and day 5 were selected into group A and group B . Endometrial thickness of the day of application progestin transformation endometrial ( Dp), estrogen total dose and continuous time before Dp, pregnancy rate, abortion rate between the two groups were compared. Relationship between endometrial thickness and outcome of FET was assessed. Results: The endometrial thickness of Dp in group A and group B were (8.8 ± 1.23) mm and (9.14 ± 1.40) mm, estrogen total dose and continuous time before Dp in group A were (83.97 ±37.5) mg and ( 15.83 ±3.01 ) d, in group B were (67.42 ±6. 19) mg and ( 11.40 ± 2. 81 ) d. There were significant differences between the two groups ( P 〈 0. 05 ) . The pregnancy rate and abortion rate in group A were 45.1% and 6. 1%, in group B were 47. 1% and 6. 7%. There were no significant differences between the two groups (P 〉0. 05) . Conclusion: There is no effect of artificial endometrial stimulation protocols with different starting time of estrogen on outcome of FET. Method B can achieve more perfect endometrial thickness, simulate the physiology, and be beneficial to the growth of the endometrium.
