摘要
目的:研究缬沙坦(Val)合并阿托伐他汀(Ato)对心肾综合征(CRS)大鼠心肾功能的影响及机制。方法:SD大鼠经"两次手术切除法"行3/4肾切除,1周后皮下注射异丙肾(100 mg/kg,分2次,间隔24 h),4周后将存活大鼠随机分为对照组(Control组)、Val组、Ato组和Val+Ato组,试剂盒法测定血清肌酐(Cr)、尿素氮(BUN)、B型尿钠肽(BNP)、醛固酮(ALD)、C-反应蛋白(CRP),Bradford法测定24 h尿蛋白量。治疗组给予Ator或(和)Val[10 mg/(kg.d)、80 mg/(kg.d)]、Control组给予生理盐水灌胃,干预4周后重新检测上述血清指标,左心室插管术测定左室收缩压(LVSP)、左室舒张压(LVDP)、左室舒张末压(LVEDP)及左室压力最大上升及下降速率(±dp/dtmax)。称重法测定心脏重量参数。切片HE染色观测心肌病理情况。结果:与Control组相比,各治疗组大鼠血清Cr、BUN、BNP、ALD、CRP、尿蛋白明显降低,LVSP上升,LVDP、LVEDP及心脏重量指数下降,差异有统计学差异。而Val+Ato组变化最显著。结论:缬沙坦与阿托伐他汀合用能更加有效保护CRS大鼠心肾功能。
Objective: To investigate the effects of combined treatment with Valsartan and Atorvastatin on chronic heart failure with renal failure in rats.Methods: Sprague-Dawley(SD) rats received three-quarters nephrectomy by twice resections,and after one week,the rats were subcutaneously injected with isoproterenol(100 mg/kg,twice injections,with a 24 hour interval).Four weeks later,survival rats were randomly separated into four groups as control group,Valsartan group(Valsartan 80 mg/kg per day),Atorvastatin group(Atorvastatin 10mg/kg per day),and Val + Ato group(Valsartan 80 mg/kg per day, Atorvastatin 10 mg/kg per day).Serum creatinine(Cr),blood urea nitrogen(BUN),B-type natriuretic protein(BNP),aldolase(ALD),and C-reactive protein(CRP) were determined by kit assay.24 h urine protein was determined by the Bradford method.After Four weeks,the serum and urine protein were determined repeatedly,LVSP,LVDP,LVEDP,dp/dtmax and-dp/dtmax were determined by left ventricular intubation.Heart weight indices were determined and the myocardial pathological conditions were observed by hematoxylin and eosin(HE) staining.Results: Compared with the control group,each index of serum and urine protein in the treatment group was significantly decreased,Additionally,LVDP, LVEDP,and heart weight index were decreased,while LVSP and dp/dtmax were increased markedly(P 0.05).Val + Ato group showed more changes than single treatment group.Conclusion: Combination of Valsartan and Atorvastatin are more effective on chronic heart failure and renal failure in rats than Valsartan or Atorvastatin administrates alone.
