摘要
目的 :观察依贝沙坦 (irbesartan)对充血性心力衰竭(congestiveheartfailure ,CHF)患者心功能的影响 .方法 :按入院时间随机分为治疗组 (A组 ,n =30 )及对照组 (B组 ,n =30 ) .两组常规使用洋地黄、利尿剂、血管扩张剂 .A组加用依贝沙坦 75～ 1 5 0mg·d-1 ,B组加用卡托普利 2 5mg 3·d-1 ,治疗 4～ 6wk ,观察治疗前后心率、血压、心胸比、左室舒张压未期内径 ,左室射血分数以及心功能的变化 .结果 :A组治疗后心率 (次·min-1 ) (1 1 5± 9)vs (83± 1 1 ) ,血压 (kPa)SP :(1 7.4± 1 .8)vs(1 3.4± 1 .5 ) ,DP :(1 1 .5± 0 .8)vs (9.4± 0 .6 ) ;心胸比 (0 .73± 0 .0 5 ) %vs (0 .5 2± 0 .0 3) %以及左室舒张压未期内径 (6 5 .3± 6 .8)mmvs (5 0 .7± 3.9)mm较治疗前均显著下降 (P <0 .0 5 ,P <0 .0 1 ) ,左室射血分数 (0 .38± 0 .0 3) %vs (0 .6 0± 0 .0 5 ) %较治疗前增加 (P <0 .0 1 ) ,心功能改善 .A组治疗后各项指标较B组差异显著 (P <0 .0 5 ,P <0 .0 1 ) .结论 :依贝沙坦对CHF患者心功能有显著改善 ,副作用小 ,值得临床推广 .
AIM: To observe the effects of irbsartan in treating patients with congestive heart failure. METHODS: The patients were randomized into therapeutic group A ( n =30) and control group B ( n =30) according to the admission date. The patients in the two groups with congestive heart failure were conventionally treated with such medicines as digitalis, diuretic and vasodilator. Meanwhile irbsartan was given to group A and captopril was given to group B (25 mg, 3·d -1 ) and the pre and post treatment changes were observed of heart rate (HR), blood pressure (BP), cardiothoracic ratio, left ventricular end diastolic diameter (LVEDD), left ventricular ejection fraction and cardiac function after 4～6 weeks' therapy. RESULTS: After being treated for 4～6 weeks, HR (115±9) vs (83±11), BP (SP: (17.4±1.8) vs (13.4±1.5), DP: (11.5±0.8) vs (9.4±0.6), cardiothoracicratio (0.73±0.05) vs (0.52±0.03) and LVEDD of the patients in group A (65.3±6.8) mm vs (50.7±3.9) mm were all obviously reduced ( P < 0.05 , P <0.01), while their left ventricular ejection fraction (0.38±0.03)% vs (0.60±0.05) % was increased ( P < 0.01 ). The cardiac function was improved as compared with that of pro treatment. All the indexes in group A were significantly different as compared with those in group B ( P < 0.05, P <0.01). CONCLUSION: Irbsartan, with few side effects, can remarkably improve the heart function of the patients with congestive heart failure.
