摘要
目的讨论膀胱副神经节瘤的临床表现和诊治方法。方法报道1例膀胱恶性非功能性副神经节瘤,对其完善影像学、组织学检查及相关诊疗,同时回顾国内外相关文献。结果膀胱副神经节瘤的典型临床症状为排尿引起的高血压危象。免疫组化染色表达CgA、Syn、Vimentin、S-100蛋白和NSE。本例患者经膀胱镜活检确诊,并发现有多处骨转移,给以CVD方案化疗。结论膀胱副神经节瘤在临床上较少见,尤其是恶性膀胱非功能性副神经节瘤更为罕见。现在在组织学上仍无法区分其良恶性,患者需长期随访。
Objective To discuss the clinical manifestations,diagnosis and treatment of paraganglioma of urinary bladder without typical symptoms. Methods A case of malignant paraganglioma of urinary bladder was observed with imageology, histology and diagnostics. The literature was reviewed. Results The typical clinical picture of micturition-evoked hypertensive crisis assisted the correct diagnosis. Immunohistochemically, the tumor cells were positive for CgA, Syn, Vimentin, S-100 protein and NSE. The case was diagnosed with cystoscope, and multiple metastasis was found. The treatment was CVD combined with cheotherapy. Conclusions Paraganglioma of the urinary bladder without typical symptoms is a rare tumor of benign na- ture in most cases, but can sometimes show malignant behavior. Long-term follow-up is essential since it is not histologically feasible to differentiate benign from malignant paragangliomas at present.
