阿奇霉素联合丙卡特罗治疗儿童哮喘并发肺炎支原体感染临床分析 被引量:24

Clinical efficacy of azithromycin combined with procaterol in treatment of asthma in children complicated with Mycoplasma pneumoniaeinfections

摘要 目的研究儿童哮喘并发肺炎支原体感染的发病情况及使用阿奇霉素联合丙卡特罗治疗的临床疗效与安全性,为临床治疗提供参考。方法选取2009年10月-2010年10月117例儿童哮喘者,筛选并发肺炎支原体感染患者42例;将其随机分为对照组和观察组,各21例;对照组采用红霉素肠溶胶囊联合盐酸丙卡特罗,观察组以阿奇霉素联合盐酸丙卡特罗治疗;观察两组临床疗效及治疗前后外周血白细胞数、哮喘症状、呼吸状况等,对>6岁患儿行肺功能检查(FEV1.0%,PEFR)。结果儿童哮喘患儿肺炎支原体感染率为35.9%;观察组治疗有效率为90.5%,对照组有效率为57.1%,两组差异有统计学意义(χ2=6.0,P<0.05);观察组白细胞数治疗前(11.2±3.9)×109/L、治疗后为(6.9±2.8)×109/L;对照组白细胞数治疗前(11.3±4.1)×109/L、治疗后为(7.6±3.1)×109/L,两组差异均有统计学意义(P<0.05);两组治疗方案对肺功能均有改善作用,但差异无统计学意义。结论阿奇霉素联合丙卡特罗治疗儿童哮喘并发肺炎支原体感染疗效确切。 OBJECTIVE To study the clinical efficacy and safety of azithromycin combined with procaterol in treatment of Mycoplasma pneumoniae infections in the children complicated with asthma so as to guide the clinical treatment.METHODS A total of 117 pediatric asthma patients who were treated in the hospital from Oct 2009 to Oct 2010,were enrolled in the study.Totally 42 patients were diagnosed as M.pneumoniae infections and were randomly divided into the control group and the observation group with 21 cases in each.The treatment group was treated with azithromycin and procaterol,while the control group was treated with erythromycin and procaterol.The clinical efficacy,white blood cell count(WBC),and respiratory status between the two groups were observed,and the pulmonary function examination(FEV1.0%,PEFR) was performed for the children aged more than 6 years.RESULTS The incidence of M.pneumoniae infection in the pediatric asthma patients was 35.9%,the effective rate of the treatment was 90.5% in the observation group and 57.1% in the control group,the difference between the two groups was statistically significant(χ2=6.0,P0.05).The WBC counts of the observation group were(11.2±3.9)×109/L before the treatment and(6.9±2.8)×109/L after the treatment;while the WBC counts of the control group were(11.3±4.1)×109/L before the treatment and(7.6±3.1)×109/L after the treatment,the difference between the two groups was statistically significant(P0.05);both of the treatment programs could improve the pulmonary function,but there was no statistical difference.CONCLUSION The azithromycin combined with procaterol can achieve accurate clinical efficacy in treatment of M.pneumoniae infection in pediatric patients complicated with asthma.
