摘要
目的检测EB病毒(EBv)、巨细胞病毒(CMV)、肺炎支原体(MP)和柯萨奇病毒(COX)4种病原体感染后患儿T淋巴细胞亚群和活化T淋巴细胞亚群表达情况,探讨不同病原体感染后的变化特点。方法采用流式细胞术检测病原体感染后患儿急性期外周血CD3+、CD4+、CD8+、CD3+HLA-DR+、CD3+CD25+、CD8+HLA-DR+、CD8+CD38+和CD4+CD25+T淋巴细胞表达情况,并与健康儿童对照比较。结果 EB感染后,患儿急性期CD3+、CD8+、CD3+HLA-DR+、CD8+HLA-DR+和CD8+CD38+T细胞比例明显增高;CD4+、CD4+/CD8+和CD4+CD25+T细胞亚群显著降低;CD3+HLA-DR+、CD8+HLA-DR+、CD8+CD38+细胞比例与患儿体温呈负相关,CD3+CD25+与患儿的发热时间呈正相关;其他病原体感染中,CMV感染患儿CD4+细胞比例显著高于MP、COX感染患儿和正常儿童,CD4+/CD8+显著高于MP感染患儿和正常儿童,CD8+CD38+细胞比例显著高于MP感染患儿和正常儿童;MP感染患儿急性期淋巴细胞CD3+细胞比例显著低于正常儿童,CD8+CD38+细胞比例显著低于COX感染患儿。结论 4种病原体感染后患儿体内呈免疫活化和失衡状态;不同病原感染后,患儿T细胞免疫改变各具特点;检测患儿T淋巴细胞亚群和活化T淋巴细胞亚群可作为一种鉴别和监测手段,为临床区别该4种病原体感染和掌握患儿病情提供重要参考依据。
OBJECTIVE To observe the expression of T lymphocytes and the activated T lymphocytes of the children infected with four species of pathogens including Epstein-Barr virus(EBv),cytomegalovirus(CMV),Mycoplasma pneumoniae(MP),and Coxsackie virus(COX) and to explore the characteristics of the variations after being infected with different pathogens.METHODS Flow cytometry was employed to detect the expression of CD3+,CD4+,CD8+,CD3+HLA-DR+,CD3+CD25+,CD8+HLA-DR+,CD8+CD38+ and CD4+CD25+ of the T lymphocytes in the peripheral blood at acute stage of the children infected with the pathogens and to compared with the healthy children as controls.RESULTS After the EB infection,the proportion of CD3+,CD8+,CD3+HLA-DR+,CD8+HLA-DR+,or CD8+CD38+ of T lymphocytes at acute stage was significantly increased;the proportion of CD4+,CD4+/CD8+,or CD4+CD25+ of the T lymphocytes was significantly decreased;the proportion of CD3+HLA-DR+,CD8+HLA-DR+,or CD8+CD38+ was negatively correlated with the body temperature of the children,while the proportion of CD3+CD25+ was positively correlated with the fever course.Among the children infected with other pathogens,the children infected with CMV took lager proportion of CD4+,CD4+/CD8+ or CD8+CD38+than did the children infected with MP or the healthy children;while the children infected with MP at acute stage took significantly lower proportion of CD3+ than did the healthy children,with the proportion of CD8+CD38+significantly lower than that of the children infected with COX.CONCLUSION The immune system of the children infected with the four species of pathogens was in an activated and imbalanced status;after being infected with the pathogens,the immunities of the T lymphocytes in the children varied in the characteristics of their own;the detection of T lymphocytes subsets and the activated T lymphocytes subsets can be appropriately for the identification and the monitoring,offering important referenc
