摘要
目的总结彩色多普勒超声对肾动脉狭窄介入治疗前后血流参数结果以评价介入治疗效果。方法选择经肾动脉造影证实肾动脉狭窄并行肾动脉血管成形术成功患者30例,对介入治疗前后血流参数、血压及肾功能进行分析。结果介入治疗后狭窄肾动脉的峰值流速(peakflowvelocity,Vp)由治疗前(279.6±94.3)cm/s降至(145.5±33.5)cm/s,肾内段动脉血流加速度(acceleration,AC)由(108.5±32.5)cm/s2增至(366.5±120.8)cm/s2,肾内段动脉血流加速时间(accelerationtime,AT)从术前(0.067±0.054)s降至(0.035±0.032)s,治疗前后以上指标差异均有统计学意义(P<0.05);Rl术前为(0.72±0.21),术后为(0.70±0.45),治疗前后差异无统计学意义(P>0.05)。术后6个月患者的平均收缩压由术前(26.35±2.13)kPa降至(20.29±2.45)kPa,平均舒张压由术前(16.43±2.51)kPa降至(13.25±2.27)kPa,与术前比较差异有统计学意义(P<0.05)。血肌酐由术前(145.5±28.6)μmol/L降至(133.1±23.8)μmoI/L,但与术前比较差异无统计学意义(P>0.05)。结论彩色多普勒超声是评价肾动脉狭窄介入治疗效果的一种无创、简便、可靠的检查方法。
Objective To evaluate the hemodynamic changes during the Interventional treatment of renal artery stenosis by Color Doppler Flow Imaging. Methods The blood flow parameters, blood pressure, kidney function of 30 cases with renal artery stenosis which were confirmed by renal artery angiography were compared and analyzed before and after the treatment. Results The Vp of the main renal artery decreased from (279.6±94.3)cm/s to (145.5±33.5)cm/s, the AT in intra-renal decreased from (0.067±0.054)s to (0.035±0.032)s, the AC increased from (108.5±32.5) cm/s 2 to (366.5±120.8) cm/s 2 , there was significant difference in these parameters before and after treatment (P 0.05). The Rl was (0.72±0.21) before intervention and was (0.70±0.54) after intervention, the difference was significant (P 0.05). The average systolic blood pressure decreased from (26.35±2.13)Kpa to (20.29±2.45)Kpa, and the average diastolic blood pressure decreased from (16.43±2.51)Kpa to (13.25±2.27)Kpa after stent treatment (P 0.05). The serum creatinine was (145.5±28.6)μmol/L before angioplasty, and it was (133.1±23.8)μmoI/L 6 months after treatment, there was no significant difference between the two groups. Conclusion Color Doppler ultrasound is a noninvasive, convenient, reliable method to evaluate the effect of intervention treatment of renal artery stenosis.
