摘要
目的对"榫头式"髂骨瓣血管蒂移植内固定法治疗青壮年不稳定型骨折的临床疗效进行研究,证实该法的临床价值。方法以我院2008年4月至2009年4月收治的青少年不稳定型股骨颈骨折患者46例为研究对象,将其随机分为观察组和对照组,每组23例。对照组患者使用经皮多钉进行固定,观察组患者使用"榫头式"髂骨瓣血管蒂移植内固定法进行固定。对两组患者治疗后的下床活动时间及随访3年骨折愈合情况进行比较和统计学分析。结果观察组患者术后平均下床扶拐不负重活动时间为4.8周,平均弃拐活动时间14.8周;随访3年骨折愈合率95.7%,无股骨头坏死病例;观察组各项数据均优于对照组,且具有显著差异性(P<0.05)。结论 "榫头式"髂骨瓣血管蒂移植内固定在青壮年不稳定型股骨颈骨折的治疗中较三翼钉固定具有愈合率高、恢复时间快、并发症少等优势,值得临床推广使用。
Objective To study the clinical curative effect of ＂tenon type＂ of iliac bone flap with vascular pedicle graft fixation in treatment of unstable fractures of the young, and confirm the clinical value of this method. Methods 46 patients with unstable femoral neck fracture in our hospital from April 2008 to April 2009 were selected as the research object and randomly divided into the control group and the observation group, with 23 cases in each group. The patients in the control group received the percutaneous screw fixation, while patients in the observation received the ＂tenon type＂ of iliac bone flap with vascular pedicle graft fixation. The time movement out of bed after treatment and the fracture healing situation during 3 years＇ follow-up of two groups were compared and analyzed statistically. Results The mean time movement out of bed with support and no weight bearing was 4.8 weeks, and the time without support was 14.8 weeks, the fracture healing rate was 95.7% without cases of femoral head necrosis in the observation group. The data in the observation group were superior to the control group, and the differences between two groups were statistically significant （P 〈0.05）. Conclusions ＂Tenon type＂ of iliac bone flap with vascular pedicle graft fixation in the treatment of unstable fracture in adolescent has the advantages of high healing rate, fast recovery time, fewer complications compared with the trifin nails fixation, which is worthy of clinical application.
