摘要
目的分析代谢综合征(MS)的临床及生化特征,探讨外周血白细胞与代谢综合征之间的关系。方法采用病例对照的研究方法,在2009年4~10月于宁夏医科大学附属医院进行的宁夏职业人群MS流行病学基线调查人群中,按照MS病例组与对照组的纳入与排除标准,确定MS病例组687例,对照组1 359例,比较两组人群外周血白细胞水平及异常率以及体质指数(BMI)、腰围(WC)、收缩压(SBP)、舒张压(DBP)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)和空腹血糖(FPG)水平,分析白细胞计数与BMI、WC、SBP、DBP、TC、TG、HDL-C、LDL-C和FPG水平的相关性,比较不同代谢综合征组分组白细胞计数的高低,分析白细胞水平与代谢综合征各种组分之间的关系。结果 MS组白细胞水平明显高于对照组,差异有显著性(t=7.777,P〈0.01);MS组白细胞异常率为12.95%,对照组为8.98%,MS组显著高于对照组(χ2=7.806,P〈0.01);MS组BMI、WC、SBP、DBP、TC、TG、HDL-C和FPG水平均明显高于对照组(P〈0.01);随着所含MS组分的增加,WBC计数水平逐渐增高(F=20.539,P〈0.01),无代谢异常组与含有代谢异常组之间差异均有显著性(均P〈0.01)。WBC异常率随代谢紊乱的加重而增加,差异有显著性(χ2=13.137,P〈0.05)。白细胞水平与BMI、WC、SBP、DBP、TC、TG和UA呈正相关(r值分别为0.207、0.227、0.122、0.141、0.055、0.191和0.173,P〈0.01),与年龄、HDL-C呈负相关(r值为-0.139和-0.162,P〈0.01);多因素Logistic回归分析结果显示,WBC计数水平为危险因素(P〈0.05)。结论 MS者WBC计数明显高于非MS者,WBC计数与MS呈正相关关系,且随着代谢紊乱的加重而增加,外周血WBC水平可以作为MS慢性亚临床炎症的反映指标之一。
【Objective】 To analyze the metabolic syndrome(MS) clinical and biochemical characteristics,and to explore the relationship between white blood cell and the metabolic syndrome.【Methods】 Collected data of the medical examination of personnel enterprises clinical in April 2009 to September in the Affiliated Hospital of Ningxia Medical College in Yinchuan City were analyzed using case-control study method,according to the inclusion and exclusion criteria of MS patients and control groups to determine the MS group(case group) of 687 cases and non-MS group(control group) of 1 359 cases.The white blood cell levels and the prevalence of hyperuricemia were compared,the relationship of body mass index(BMI),waist circumference(WC),systolic blood pressure(SBP),diastolic blood pressure(DBP),total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C) and fasting plasma glucose(FPG) level were analyzed for two groups.【Results】 The white blood cell value of MS group was higher than that of non-MS group,there was a significant difference(t =7.777,P 0.01);MS group,the abnormal level of white blood cell was 12.95%,that of non-MS group was 8.98%,MS group was significantly higher than the non-MS group(χ2 =7.806,P 0.01);BMI,WC,SBP,DBP,TC,TG,HDL-C,FPG of MS group were significantly higher than that of non-MS group(P 0.01);Along with the increase of components with MS,WBC count level was higher(F =20.539,P 0.01),there was a significant difference in the non metabolic abnormalities group and containing the metabolic abnormalities group.WBC abnormal rate varied with the increase of the metabolic disorder,their difference was statistically significant(χ2 =13.137,P 0.05);and white blood cell levels was positively correlated with BMI,WC,SBP,DBP,TC,TG,UA(r values were 0.207,0.227,0.122,0.141,0.055,0.191,0.173,P 0.05),and was a negative correlative with age,HDL-C(r value of-0.139,-0.162,P 0.0
