摘要 目的探讨高原低氧环境对成年男性精液质量及血清生殖激素的影响。方法采用自身前后对照方法,对同一批进驻5380米高原某部男性军人,于上山前一个月、上山后半年、上山后一年和下山后半年4个时间段,分别观察男性精液质量及血清生殖激素水平的变化。精液分析采用计算机辅助分析系统进行分析,血清生殖激素水平测定采用ELISA法。结果精液质量检测发现,上山半年与上山前相比,精子总数、精子密度、精子活力和精子活率显著降低,精液液化时间显著延长;上山一年与上山前相比,精子活力和精子活率进一步明显减低,但精子密度大幅回升;下山半年后,除液化时间仍然明显延长之外,其他指标均己恢复。血清生殖激素测定显示,上山半年后卵泡刺激素(FSH)显著升高,而黄体生成素(LH)、泌乳素(PRL)和睾酮(T)均显著降低。上山一年时,FSH大幅回落甚至低于上山前水平,LH继续维持低水平,但PRL有所恢复,而T显著升高。下山半年后,T和PRL仍处于高值,但LH和FSH基本恢复至正常水平。结论急进高原时,低氧环境对男性精液质量有严重影响,对机体生殖激素的分泌与调节有不同程度的干扰,但这些不良影响在脱离低氧暴露环境后可逐渐恢复。 Objective To investigate the effects of hypoxic environment at high altitude on semen quality and serum reproductive hormones of adult male. Methods Using a strategy ofpre and post self-control test, semen quality and levels of serum reproductive hormones of same soldiers were comparatively analyzed at one month before, six months after, one year after staying on the high altitude and six months after leave the high altitude, respectively. Levels of serum reproductive hormones were detected by ELISA and general parameters of semen specimens were analyzed by computer-assisted sperm analysis system(CASA). Results Semen quality testing showed that these soldiers staying on high altitude for half a year compared with that before staying, the total number of sperm, sperm density, sperm motility and survival rate of sperm were significantly decreased and semen liquefaction time wassignificantly prolonged. Staying on high altitude for one year compared with that before staying, the sperm motility and survival rate of the soldiers were further reduced, but the sperm density was increased sharply. All indexes were in normal value range except semen liquefaction time at the sixth months after leave the high altitude. The level of FSH was significantly increased and the levels of LH, PRL and T were significantly decreased about half a year after staying on high altitude. When staying on plateau for one year, LH was still at a low level and FSH was decreased sharply, even lower the level before staying. PRL level was restored gradualy and T was still increased obviously after one year staying. After leave the high altitude for half a year, T and PRL were still at a high level, but LH and FSH were all restored. Conclusion When rapidly entering high altitude pateau, hypoxia environment can cause serious negative effects on sperm quality of adult male and disturb the secretion and regulation of reproductive hormones to differen extent. These negative influence would gradually restored after leaving hypaxia exposure environm
