频域光相干断层扫描观察特发性黄斑裂孔手术前后光感受器内外节的变化 被引量:1

Observation of the changes of photoreceptor inner /outer segment junctions before and after surgery in patients with idiopathic macular hole by spectral domain optical coherence tomography

摘要 目的:通过频域光相干断层扫描(SD-OCT)观察光感受器内外节在特发性黄斑裂孔手术前后的变化。方法:前瞻性对比分析23例(25只眼)确诊并接受玻璃体手术治疗的特发性黄斑裂孔患者的临床资料,采用Topcon公司Version3.21型SD-OCT测量并对比分析手术前后水平方向上光感受器内外节缺失区直径(diameter of IS/OS absence,DIOA)的变化。结果:手术后有18只眼黄斑裂孔闭合,裂孔封闭率为72%。术前DIOA范围为551~2900μm,平均(1933.16±650.80)μm;术后12个月DIOA范围为412~2811μm,平均(1329.20±780.13)μm。经配对样本的t检验,术前DIOA与术后DIOA之间不存在相关关系(r=0.37,P=0.069),手术前后差异有显著性(t=3.73,P=0.001)。结论:SD-OCT能清晰观察视网膜IS/OS层的结构,特发性黄斑裂孔患者术后水平方向上光感受器内外节缺失区直径较术前明显减少。 Objective To evaluate the changes of photoreceptor inner/outer segment (IS/OS) junctions before and after surgery in patients with idiopathic macular hole (IMH) by spectral domain optical coherence tomography (SD-OCT). Methods Data of 23 patients (25 eyes) with IMH, who underwent macular hole surgery were prospectively analyzed. Date of pre- and postoperative diameters of IS/OS absence (DIOA) in horizontal direction were collected. Results Macular holes in 18 eyes (72%) had successful anatomic closure. The mean DIOA was decreased from (1 933.16 ± 650.80)μm to (1 329.20 ± 780.13)μm. The pre- and postoperative DIOA had no correlation (r = 0.37, P = 0.069), but they did have significant difference (t = 3.73,P = 0.001 ). Conclusions SD-OCT can be valuable for the observation of IS/OS junctions in IMH patients and postoperative DIOA was lower than preoperative significantly.
