摘要
目的 :观察足三里穴位注射防止胆囊牵拉反应的效果。方法 :2 4 0例行胆囊切除术患者 ,随机分为 4组 ,每组 6 0例。术前 2 0min ,治疗A组用 10 %葡萄糖液 4ml加杜冷丁 50mg行双侧足三里穴位注射、治疗B组用 10 %葡萄糖液 4ml行双侧足三里穴位注射、阳性对照组肌注杜冷丁50mg、阴性对照组术中酌情用药 ,详细记录术中牵拉反应 ,观察呼吸、血压、心率、氧饱和度变化。结果 :预防牵拉反应优良率 ,治疗A、B组显著高于阴性对照组 (P <0 .0 1) ,阳性对照组也高于阴性对照组 (P <0 .0 5)。 4组病人呼吸、血压、心率、氧饱和度相对平稳。结论 :穴位注射可抑制术中胆囊牵拉反应 ,减轻患者痛苦 ,利于麻醉管理。
Objective:To investigate the effect of hydro acupuncturing Zusanli point on pulling dragging reflex during cholecystectomy.Method:240 cases of cholecystectomy were divided into 4 groups at random.20 minutes before operation,patients in treatment group A were hydro acupunctured at Zusanli point with 10% glucose 4ml and dolantin 50mg,while treatment group B with dolantin 50mg alone.Positive contrast group were injected intramuscularly with dolantin 50mg,and negnative contrast group without any drugs unless necessary during operation.The change of respiration rate,heart rate,blood pressure,partial pressure of oxygen were recorded and analyzed.Result:The effects of treatment group A,B and positive contrast group were better than those of negative contrast group in aspect of preventing pulling dragging reflex of gallbladder(P<0.01,P<0.05 respectively).Conclusion:Hydro acupuncture therapy can inhibit pulling dragging reflex during cholecystectomy,relieve pains and be advantage to anesthetic management.
