摘要
目的 观察硬膜外麻醉下辅用适量度冷丁、力月西对胆囊切除术时胆囊牵拉反应的抑制效果。方法 80例在硬膜外麻醉下胆囊单纯切除术的ASAⅠ～Ⅱ级病人 ,随机配对分为两组 ,每组 4 0例 ,术前 10min ,分别静注度冷丁加力月西或芬太尼加力月西作为硬膜外麻醉的辅助用药 ,观察二者对胆囊牵拉反应的抑制效果 ,并行比较。结果 度力组对胆囊牵拉反应的抑制效果与镇静效果均优于芬力组 (P <0 .0 1) ,对呼吸、循环影响无显著差异 (P >0 .0 5 )。结论 度冷丁、力月西联合应用 ,抑制硬膜外麻醉下胆囊牵拉反应的效果满意 ,减轻患者痛苦 ,利于麻醉管理。
Objective To investigate the preventing effect of dolantin-rhythim on pulling-dragging reflex during cholecystectomy.Methods 80 ASAⅠ～Ⅱ patients aged 25～60 undergoing cholecystectomy singly under epidural anesthesia were divided into 2 groups randomly and evenly: the dolantin-thythim (DR) group and the fentanyl-rhythim group. 10 minutes before operation,patients in the DR group were injected with dolantin 50mg and rhythim 5mg while in the FR group with fentanyl 0.05mg and rhythim 5mg. The changes of the pulse oxygen saturation, the respiration rate(RR), the heart rate(HR), the blood pressure (BP) and the situation of preventing pulling-dragging reflex were recorded and analyzed. Results The effect of the DR group was better than that of the FR group in aspects of preventing pulling-dragging reflex of gallbladder and sedation (P<0.01, P<0.01). The effect of respiration and circulation were not different (P>0.05).Conclusion Dolantin and thythim intravenously injected can inhibit pulling-dragging reflex of gallbladder during cholecystectomy, relieve pain and be helpful to anesthetic management.
