摘要
目的:观察葛芪降糖颗粒对2型糖尿病周围神经病变(DPN)多元醇途径活性的影响,以探讨葛芪降糖颗粒作为醛糖还原酶(AR)抑制剂防治的临床意义。方法:选用我院诊断为DPN且中医辨证为肾虚血瘀、毒损脉络型的患者30例,按随机、单盲原则分为治疗组和对照组各15例,两组均进行基础治疗2周,对照组在基础治疗后运用甲钴胺治疗,治疗组在对照组治疗基础上运用葛芪降糖颗粒治疗。两组均治疗8周后比较疗效,观察治疗前后DPN患者的症状与体征;检测血糖、糖化血红蛋白(HbA1c)、AR活性;监测肌电图神经传导速度(NCV)。结果:总有效率治疗组为93.3%,对照组为73.3%,两组比较,差异有统计学意义(P<0.05);治疗组红细胞AR活性受到抑制,明显低于对照组(P<0.05),空腹血糖、负荷后2h血糖及糖化血红蛋白控制目标明显优于对照组(P<0.05);NCV监测结果比较,治疗组优于对照组(P<0.05)。结论:葛芪降糖颗粒可明显抑制DPN患者的AR活性,改善DPN患者的症状和体征,有助于血糖的控制。
Objective：To observe the effect of Geqi Jiangtang granules（for lowering blood glucose（BG）） on the polyol pathway activity in type 2 diabetes patients with diabetic peripheral neuropathy（DPN）,and to investigate the clinical significance of Geqi Jiangtang granules as an aldose reductase（AR） inhibitor in the prevention and treatment of DPN.Methods：Thirty patients,who were diagnosed with DPN and traditional Chinese medicine syndromes of deficiency of the kidney,blood stasis,and toxin impairing collaterals in our hospital,were divided into treatment group（n=15） and control group（n=15） in a randomized,single-blinded trial.Both groups received basic treatment for 2 weeks;then,the control group received mecobalamin,and the treatment group received Geqi Jiangtang granules in addition to mecobalamin.After 8 weeks of treatment,the therapeutic effects were compared.The symptoms and signs of DPN patients were observed before and after treatment;the BG,glycosylated hemoglobin（HbA1c）,and AR activity were measured,and the nerve conduction velocity（NCV） was monitored by electromyography.Results：Compared with the control group,the treatment group showed significantly higher total effective rate（93.3% vs 73.3%,P〈0.05）,significantly lower AR activity in red blood cells（P〈0.05）,significantly better control of fasting BG,BG at 2 h after glucose load,and HbA1c（P〈0.05）,and significantly more improvement in NCV（P〈0.05）.Conclusion：Geqi Jiangtang granules can markedly inhibit the AR activity and improve the symptoms and signs in DPN patients,thus helping control BG.
