摘要
目的探讨慢性阻塞性肺疾病（COPD）患者健康行为与社会支持的相关性。方法采用便利抽样的方法，选取广州市1家三级甲等医院病情稳定准许出院的和门诊治疗的69例COPD患者为调查对象，采用一般资料问卷、健康行为量表（health promotion lifestyle，HPL）和社会支持量表（social support research scale，SSRS）对患者的健康行为与社会支持进行测评。结果69例COPD患者社会支持总分为（30．580±5．097）分，社会支持程度均处于中等水平；健康行为总分为（130．710±16．348）分，健康责任感、躯体活动、营养、心理健康、人际关系和压力调解6个维度得分均低于33％，为低等水平。69例COPD患者社会支持的各维度得分与健康行为的健康责任感、躯体活动、营养、心理健康及压力调解各维度评分均存在相关性（r值为0．239～0．555，P〈0．05）；而社会支持的客观支持、支持的利用度维度得分与健康行为的人际关系维度尚不存在相关性（P〉0．05）；COPD患者的健康行为总分与社会支持总分存在显著相关性（r=0．371，P〈0．01）。结论COPD患者健康行为与社会支持呈正相关，提高患者的社会支持，实施有针对性的护理干预可提高患者的健康行为，提示今后改善COPD患者健康行为的状况，可以从提高患者社会支持的水平着手。
Objective To study the correlation between health behavior and social support of patients with chronic obstructive pulmonary disease （COPD）. Methods Convenience sampling method was used to select 69 cases of COPD in a Class 3 hospital in Guangzhou who were stable enough to be permitted for discharge and ambulatory treatment. General information questionnaire, health promotion lifestyle （HPL） and social support research scale （SSRS） were used to assess their health behavior and social support. Results Total score of the 69 COPD patients＇ social support was （30. 580 ＋ 5. 097）, which was medium. And total score of their health behavior was （130. 710 -＋ 16. 348 ）, in which scores of health responsibility, body activity, nutrition, metal health, interpersonal relationship and stress adjustment were all lower than 33%, which was the low level. There was statistically significant difference between every dimension of social support and dimensions in health behavior including health responsibility, body activity, nutrition, metal health and stress adjustment （r =0. 239 -0. 533, P 〈 0.05 ）. There was no statistically significant difference between objective support, support availability in social support and interpersonal relationship in health behavior （ P 〉 0.05 ）. The total score of COPD patients＇ health behavior was related to the total score of their social support （ r = 0. 371, P 〈 0.01 ）. Conclusions The health behavior and social support of COPD patients is positively correlated. Better social support and targeted nursing intervention can improve their health behavior. It is, therefore, an effective way to improve COPD patients＇ heath behavior by raising the level of social support.
