新柏氏薄层细胞技术在判读宫颈涂片上的应用价值

摘要 目的探讨新柏氏薄层细胞技术(TCT)在判读宫颈涂片上的应用价值。方法采用无菌取样器采集宫颈外口及宫颈管内细胞158例,采用传统巴氏染色制作宫颈涂片作为对照组;经新柏氏ThinPrep 2000全自动细胞制片机(ThinPrep2000)处理器制片,然后进行巴氏染色作为实验组。TCT检查使用伯塞斯达系统(TBS)诊断,将二者染色效果和判读结果进行对比。结果参照国际妇产科联盟(FIGO2009)最新修订的临床分期进行结果分类,对照组和实验组判读结果为正常者分别为132例和144例,Ⅰ期患者分别为8例和3例,Ⅱ期患者分别为6例和4例,Ⅲ期患者分别为6例和3例,Ⅳ期患者分别为6例和4例;158例患者中临床确诊为正常者148例,Ⅰ期患者2例,Ⅱ期患者为3例,Ⅲ期患者为2例,Ⅳ期3例,对照组符合率为89.2%,实验组符合率为97.3%,两组比较,差异有统计学意义(P<0.05)。结论采用TCT宫颈涂片,在涂片效果和判读结果上都较传统巴氏涂片检查更加准确、容易。 Objective To investigate the application value ofthinprep cytologic test(TCT) in cervical smear's interpre- tation. Methods 158 cases of the external cervical orifice and cervical canal cells were collected by aseptic sampler. The control group used the traditional Papanicolaou staining to prepare the cervical smears ,while the experimental group adopted Papanico- laou staining for preparing the cervical smear after precessing by ThinPrep2000. The dyeing effects and the interpretation results were compared between the two groups. Results Referring to the newly revised clinical stage classification by the FIGO2009, the normal cases were 132 in the control group and 144 in the experimental group respectively,the stage Ⅰ was in 8 cases and 3 cas- es,the stage Ⅱ was in 6 cases and 4 cases,the stagem was in 6 cases and 3 cases,and the stageⅣ was in 6 cases and 4 eases, respectively. The clinical diagnosis was normal in 148 cases ,the stage I in 2 cases ,the stage H in 3 cases and the stage Ⅲ in 2 cases ,the stage IV in 3 cases. The coincidence rate was 89.2% in the control group and 97.3% in the experimental group ,the dif- ference between them showing statistical significance (P〈0.05). Conclusion The TCT cervical smear is more accurate and easi- er in the smear effects and the result interpretation than the conventional Papanicolaou staining.
