摘要
目的:观察盐酸曲马多配合美洛昔康片超前镇痛对肩关节松动术的影响。方法:将62例肩关节功能障碍的病人按照就诊顺序交叉分为治疗组与对照组,每组31例;治疗组于关节松动术前2h给予盐酸曲马多100mg联合美洛昔康片7.5mg口服,对照组不给药物。记录5周内每位患者松动术前2h、术中和术后2h的VAS评分,测量每个患者首次治疗前和治疗5周后肩关节前屈、外展和内外旋的角度,并且评估每位患者康复治疗的依从性情况。结果:治疗组关节松动术中和术后2h的VAS平均评分低于对照组,差异有统计学意义(P<0.01)。治疗组术后5周的肩关节前屈、外展和内外旋角度改善均优于对照组,差异有统计学意义(P<0.01),治疗组患者康复治疗依从性也优于对照组,差异有统计学意义(P<0.01)。结论:盐酸曲马多联合美洛昔康片超前镇痛用于关节松动术能缓解关节松动术引起的疼痛,有利于改善肩关节的关节活动度,有利提高患者治疗的依从性,值得临床推广。
To observe the preemptive analgesic effect of tramadol hydrochloride and the meloxicam tablets applied before shoulder mobilization. Methods： Sixty-two patients with shoulder dysfunction were divided into treatment group and control group according to visiting sequence, 31 cases in each group. Patients in treatment group were given tramadol hydrochloride 100mg and meloxicam tablet 7.5 mg before the joint mobilization, while patients in control group received no medicine before the joint mobilization. Visual analogue scale （VAS） of each patient 2 hours before the mobilization, during the operation and 2 hours after the operation were respectively recorded. Shoulder joint activity was also measure in each patient before the first treatment and five weeks later, including shoulder flexion, outreach, internal and external rotation angle. Meanwhile, compliance of each patient was assessed. Results;The average intraoperative and postoperative VAS in treatment group were remarkably lower than those of control group （P〈0.01）. Obvious improvement of shoulder function was seen in treatment group compared with control group, and the difference was statistically significant （P〈0.01）. The rehabilitation compliance of the treatment group was better than the control group （P〈0.01）. Conclusion;The preemptive analgesia of tramadol hydrochloride and meloxieam tablets can alleviate the pain caused by joint mobilization, improve shoulder range of motion, and improve the compliance of patients. Therefore, this method is worthy of extensive clinical application.
