摘要
目的研究α-葡萄糖苷酶抑制剂配合胰岛素使用,对2型糖尿病患者机体水溶性维生素水平的影响。方法将73例符合要求的2型糖尿病住院患者随机抽号分为研究组和对照组,研究组37例,对照组36例,两组患者既往均未使用阿卡波糖(拜唐苹),在血糖、并发症及伴随症状方面差异无显著性;住院期间由医院临床营养科统一安排糖尿病饮食,两组饮食差异无显著性。住院期间对照组不使用阿卡波糖,研究组使用阿卡波糖,且均为初始用量50 mg tid,两组间其余用药差异无显著性。分别于住院前和住院第5天检测空腹和餐后2 h静脉血葡萄糖水平,以及血清和尿液中维生素B1、B6、B12、C和叶酸水平。结果两组在空腹血糖变化情况比较差异无显著性,而在餐后2 h血糖变化比较研究组较对照组明显降低(P<0.05)。研究组较对照组在血清维生素B1、B6、B12、C和叶酸水平显著降低(P<0.05),而尿维生素B1、B6、B12、C和叶酸水平变化差异无显著性。结论α-葡萄糖苷酶抑制剂长期使用可能会带来水溶性维生素的缺乏病及相关问题,提示对糖尿病末梢神经病变的原因多重性方面应增加一个考虑方面。
Objective To study the influence of ^-glycosidase inhibitors on the water-soluble vitamins in patients with type 2 diabetes mellitus. Methods 73 selected hospitalized patients with type 2 diabetes mellitus were ran- domly divided into the expermental group(n = 37) and control group (n = 36). The patients in the two groups had not used the Acarbose tablets. There was no obvious difference in the two groups in blood glucose, complications and accompanied symptoms. Hospital diabetic diet was uniformly arranged by clinical nutrition department. Two groups of diet has no significant difference. The experimental group used the Acarbose tablets with the initial dosage 50 mg tid/d during hospitalization, while the control group did not. There was no significant difference between the two groups in other medication usage. Vitamin B1, B6, B12, C and folic acid in both sera and urine were exam- ined before and 5 days after hospitalization. Results There was no difference in the viariation of fasting blood glu- cose in the two groups, while the viariation of 2 hours postprandial blood glucose was obvious ( P 〈 0.05 ) , the lev- els 2 hours postprandial blood glucose in the study group decreased more obviously. The levels of vitamin B1, B6, B12, C and folic acid in plasma in the study group were lower than those in the control group(P 〈0. 05). While there was no obvious difference in urine. Conclusion oL-glucosidase inhibitor in long-term usage may lead to lack of water-soluble vitamins and provoke some problems, which provides us with an additional consideration about the multiplicity of diabetic peripheral polyneuropathy.
