稳定期类风湿性关节炎患者血栓前状态与全身炎症反应关系的研究 被引量:3

Correlation of systemic inflammatory response with prethrombotic state in patients with stable rheumatoid arthrilis

摘要 目的通过检测稳定期类风湿关节炎(RA)患者TNF-(~、血小板参数及血管性血友病因子(vWF)、纤维蛋白原(Fg)、D-二聚体(D-D)水平,探讨血栓前状态与全身炎症反应关系。方法选取RA稳定期患者50例和健康对照组50例,测定两组血清中TNF-Q、血管性血友病因子、纤维蛋白原、血小板参数、D-D浓度。结果RA患者TNF-α水平为(2.85±0.4S)ug/L,显著高于正常健康志愿者(t=21.274,P〈0.01);RA患者vWF、Fg、IND水平分别为(100.57±41.14)%、(4.60±0.65)g/L和(0.33±0.12)mg/L均高于正常健康志愿者,差异有统计学意义(t=3.568、12.422、9.267,P〈0.01);RA患者TNF-α水平与血小板计数、血小板平均容积、血小板体积分布宽度、大型血小板比例、vWF、Fg、D—D水平呈正相关(r=0.372、0.668、0.585、0.617、0.434、0.653、0.640,P均〈0.01)。结论稳定期RA患者存在全身炎症反应和血栓前状态,稳定期RA患者的全身炎症反应有助于形成血栓前状态。 Objective To discuss the relation between prethrombotic state and systemic inflammatory response syndrome by examining the levels of tumor necrosis factor-α (TNF-α), platelet parameters, von willebrand factor(vWF), fibrinogen (Fg) and D-dimer (D-D) in patients with stable rheumatoid arthritis (RA). Methods The levels of TNF-a, platelet parameters, vWF, Fg and D-D were determined in 50 patients with stable RA and 50 healthy persons (control group), respectively. Results The level of TNF-α in RA group was (2.85±0.48) μg/L. There was a significant difference between RA group and control group ( t = 21.274, P 〈 0.01). The levels of vWF, Fg and D-D in RA group were ( 100.57 ± 41.14) %, (4.60 ± 0.65) g/L and (0.33 ±0.12) rag/L, which were significantly higher than those in control group( t = 3.568, 12.422, 9.267, P 〈 0. 01 ). In RA group, there were significant positive correlations of TNF- level with platelet count, mean platelet volume, platelet distribution width, platelet-large cell ratio, vWF, Fg and D-D levels(r =0.372, 0.668, 0.585, 0.617, 0.434, 0.653, 0.640, P 〈0.01). Conclusions The systemic inflammatory response syndrome with prethrombotic state exists in patients with stable RA, and contributes to the formation of prethrombotic state in stable RA.
