摘要
目的:研究阳痿确切有效的诊疗方法及诊疗规范。方法:应用循证医学方法。通过文献检索采集证据,依据Delphi分级方法确定推荐级别及推荐强度,从定义、诊断、辨证、治疗、调摄及预防六方面进行证据比较及选择。结果:将阳痿分成肝气郁结证、湿热下注证、瘀血阻络证、心脾两虚证、心肾惊恐证、肾阳不足证、阴虚精亏证、脾胃虚弱证,分别应用逍遥散、龙胆泻肝汤、少腹逐瘀汤、归脾汤、安神定志丸合六味地黄丸、右归丸、二地鳖甲煎、加味保和丸治之,并配以中成药、外用药、针灸等疗法,嘱以调摄及预防方法,形成了诊疗规范。
Objectives： To explore on the effective diagnosis and treatment for erectile dysfunction. Methods： Evidence - based medicine was used to collect evidence through literature retrieval. Delphi grading method was applied to determine the recommended grade and intensity. The comparison and selection of evidence were conducted in terms of six aspects including definition, diagnosis, differentiation of symptoms and signs, treatment and prevention. Results： Erectile dysfunction can be categorized into 8 types, namely, binding depression of liver qi, damp - heat pouring down, internal static blood obstruction of network vessel, dual vacuity of the heart and spleen, panic damage to heart and kidney, insufficiency of kidney yang, yin vacuity essence depletion, spleen - stomach vacuity. The different types were respectively treated with free wanderer powder, gentian liver - draining decoction, lesser abdomen stasis - expelling decoction, spleen - returning decoction, combination of spirit - quieting mind - stabilizing pill and six - ingredient rehmannia pill, right - restoring pill, double rehmannia turtle shell decocted, supplemented harmony - preserving pill. These herbal recipes can be also combined with acupuncture and moxibustion in the treatment of erectile dysfunction.
