摘要
目的观察温通针法联合补阳还五汤治疗气虚血瘀型缺血性卒中的临床疗效。方法将108例气虚血瘀型缺血性卒中患者随机分为3组,针刺组36例予温通针法,中药组36例予补阳还五汤,针药结合组36例予温通针法加补阳还五汤。3组均10 d为1个疗程,共治疗2个疗程。观察3组治疗前后神经功能缺损评分及血液流变学指标变化情况,统计临床疗效。结果针刺组总有效率61.1%,中药组总有效率66.7%,针刺结合组总有效率86.1%,针药结合组总有效率高于中药组、针刺组(P<0.05)。3组治疗后神经功能缺损评分及血液流变学指标全血黏度(高切)、全血黏度(低切)、血浆黏度均较本组治疗前明显改善(P<0.05),且针药结合组改善优于中药组及针刺组(P<0.05)。结论温通针法联合补阳还五汤能显著改善气虚血瘀型缺血性卒中患者的神经功能缺损情况及血液流变学指标,疗效确切。
Objective To observe the effect warming-promotion method of acupuncture combined with Buyanghuanwu decoction on ischemic stroke with qi-defeciency and blood-stasis syndrome.Methods 108 cases of ischemic stroke with qi-defeciency and blood-stasis syndrome were randomly divided into three groups,acupuncture group which was given warming-promotion method of acupuncture,traditional Chinese medicine group which was given Buyanghuanwu decoction and acupuncture combined with traditional Chinese medicine group which was given warming-promotion method of acupuncture combined with Buyanghuanwu decoction.The course was 10 days in three groups.Neurological deficit scores and high shear viscosity,low shear viscosity,plasma viscosity was observed after two courses.Results The total effective rate in acupuncture group was 61.1%,the total effective rate was 66.7% and 86.1% in traditional Chinese medicine group and acupuncture combined with traditional Chinese medicine group.There was significant difference between acupuncture combined with traditional Chinese medicine group and acupuncture group,traditional Chinese medicine group in total effective rate（P〈0.05）.The therapeutic effect of acupuncture combined with traditional Chinese medicine group was best in all groups.Neurological deficit scores and high shear viscosity,low shear viscosity,plasma viscosity after treatment were obviously improved as compared with those before treatment in three groups（P〈0.05）.The improvement in acupuncture combined with traditional Chinese medicine group was superior to that in acupuncture group,traditional Chinese medicine group（P〈0.05）.Conclusion Warming-promotion method of acupuncture combined with Buyanghuanwu decoction can significantly improve neurological deficit and hemorheology of ischemic stroke with qi-defeciency and blood-stasis syndrome and has definite effect.
