摘要
目的:通过确立科学的分类节点变量,建立符合陕西省的DGRs分组,为相关决策提供依据。方法:(1)用非参数检验Kruskal Wallis方法和多因素回归方法分析选取分组的分类节点变量;(2)用费用评价法、方差减少量和ROC曲线对分组结果的合理性进行检验和评价。结果:(1)筛选了年龄、性别、是否手术、伴随症、疾病危重度和是否主要手术6个因素作为分类节点变量进行分组;(2)费用评价法显示总的分组可以解释实际消耗费用的78.38%;方差减少量计算的结果是0.546 5,说明分组后的组间异质性较好;ROC曲线结果说明各组之间的费用存在差异。结论:选取的分类节点变量科学,分组结果合理。
Objective: To establish the Diagnosis Related Groups (DRGs) in Shaanxi Province and provide basis for the relative policies through setting up classification node variables scientifically. Methods: (1) Applying non parametric test (Kruskal Wallis method) and multivariate regression analysis to select classification node variables. (2) The cost evaluation method, variance reduction and ROC curve analysis were used to test and evaluate the reasonableness of grouping results. Results: ( 1 ) Gender, age, suffered any operation or not, complications, severity of the disease and whether a major operation were selected as classification node variables. (2) The cost evaluation method showed that the general grouping could explain 78.38% of the actual consumption cost; the variance reduction was 0.546 5, which proved that after grouping the between group heterogeneity was better than before; the result of ROC curves explained the difference between groups in price. Conclusion: The grouping results were scientific and reasonable.
