摘要
目的探讨胰十二指肠切除术后早期序贯性肠内营养支持疗效及护理体会。方法将64例胰十二指肠切除术后病人随机分为序贯肠内营养支持(SEN)组33例及全肠外营养(TPN)组31例。观察2组术后并发症发生情况及肛门排气时间。术前、术后第8 d分别测量病人体重及血清清蛋白(ALB)水平。结果 2组病人均未发生吻合口瘘、消化道出血等严重并发症。SEN组术后第1次肛门排气时间明显早于TPN组(P<0.05);SEN组术后第8 d体重及ALB水平高于TPN组(P<0.05)。结论胰十二指肠切除术后早期采用SEN较TPN更有利于改善营养状况,规范化护理措施是保障SEN顺利完成的关键。
Objective To investigate the clinical efficacy and nursing experience of the early sequential enteral nutrition support after pan- creaticoduodenectomy. Methods 64 cases of pancreaticoduodenectomy were randomly divided into two groups：sequential enteral nutrition （SEN） group （ 33 cases） and total parenteral nutrition （TPN）group （ 31 cases）. Postoperative complications and anal exhaust time were ob- served. To measure the body weight and serum albumin（ALB） level before and 8 days after pancreaticoduodenectomy. Results All patients of both groups have no serious complications, such as anastomotic fistula and hemorrhage of digestive tract. Anal exhaust time of SEN group was significantly earlier than that of TPN group（ P 〈 0.05） ; The body weight and ALB level of SEN group was significantly higher than that of TPN group（ P 〈 0.05）. Conclusion The SEN is more conductive to improving the nutritional status compared with TPN in the early time af- ter pancreaticoduodenectomy. Standardized nursing measures is the key to complete the SEN.
