摘要 目的:探讨无创正压通气治疗慢性阻塞性肺疾病急性加重期伴中重度Ⅱ型呼吸衰竭失败的危险因素。方法:对本院近年来收治的20例经无创正压通气治疗的慢性阻塞性肺疾病急性加重期伴中重度Ⅱ型呼吸衰竭失败患者的临床资料进行回顾性分析,记录患者一般情况、血气指标、生化指标、治疗评估等基本资料,进行统计学分析。结果:治疗失败的原因对比中,患者年龄大、入院时心率快、血压低、血糖高、日常生活能力评分(ADL)低、并发症发生率高均较治疗成功组明显。结论:二元Logistic回归分析显示年龄、心率、血压、血糖、ADL、并发症均为影响无创正压通气成功与否的独立危险因素,因此临床上可据此采取针对性干预措施。 Objective: To investigate the effect of noninvasive positive pressure ventilation in the treat- ment of acute exacerbations of chronic obstru type respiratory failure risk factors of failure. ctive pulmonary disease patients with moderate and severe II Method: Clinical data of 20 cases treated in recent years by noninvasive positive pressure ventilation in the treatment of acute exacerbations of chronic obstructive pulmo- nary disease patients with severe respiratory failure in type II in patients were analyzed retrospectively recor- ded the patients in general, blood biochemical index, assessment, treatment and other basic information. Result: The causes of treatment failure in contrast, patients with old age, Faster heart rate at the time of ad- mission, hypotension, high blood sugar, lower activities of daily living score ( ADL }, high incidence of complications were successfully treated group were. Conclusion: It showed that age, heart rate, blood pres- sure, blood glucose, ADL, complications are the effects of noninvasive positive pressure ventilation in the success of independent risk factors, therefore clinically available accordingly for intervention measures.
