血浆表达CD44分子几种基因拼接体的临床诊断价值 被引量:6

摘要 探讨血浆中可溶性 CD44标准型、v6和 v7- v8变构体的表达水平与四类肺癌的发生和转移的关系。方法 :用我们自己建立的通用免疫杂交法定量测定 5 9例肺癌患者血浆 s CD44 S、s CD44 v6、s CD44 v7- v8的含量。结果 :原发性肺癌 (5 2例 )和转移性肺癌 (7例 )的 s CD44 v6表达水平明显高于正常对照组 (P<0 .0 0 1)和肺慢性炎症对照组 (P<0 .0 5 ) ;原发性未转移组 (2 2例 )与原发性转移组 (30例 )比较 ,差异不显著 (P>0 .0 5 )。未转移组鳞癌的 s CD44 S检测水平低于正常水平 (P<0 .0 5 ) ,与肺泡癌比较差异有显著性 (P<0 .0 5 )。四类肺癌组 s CD44 v6均极显著高于正常组 ,肺泡癌与小细胞癌比较差异有显著性 (P<0 .0 5 )。s CD44 v7- v8在肺癌中无特别意义。结论 :s CD44 v6与肺癌的恶性增殖和转移密切相关 ,s CD44 v6可作为肺癌的早期诊断 ,疗效评价 ,转移和复发的监测指标。 Objective:To explore relationship between the level of plasma soluble CD44S、CD44v6、v7 v8 variant and the genesis and the metastasis of four types of lung carcinomas(SCC、ADC、SCLC、BAC).Methods:Concentration of plasma soluble CD44S and CD44v6、CD44v7 v8 in 59 lung cancer patients was measured with self established enzyme combinat immune cross methods.Results:sCD44v6 concentration of primary lung carcinoma(52 cases)and secondary lung carcinoma(7 cases)were significantly higher than that of normal control group(P<0.001)and that of chronic pneumonia group(P<0.05). However, no significant difference was noted between the primary non metastasis group(22 cases)and the primary metastasis group (30 cases).sCD44S level of the non metastasis SCC was significantly lower than nomal(P<0.05)and BAC(P<0.05).sCD44v6 of four types of lung carcinomas were all significantly higher than that of normal control, express the topotaxis that BAC level was lower SCC and ADC,SCC and ADC were lower SCLC level.There is significant differcnce between BAC level and SCLC level (P<0.05). sCD44v7 v8 is of no significant importance in lung carcinoma .Conclusion:sCD44 v6 is closely related to the malignant profiferation and the metastasis of lung carcinoma. sCD44v6 may be used as a valuable index to the early diagnosis、 evaluation of treatment,observation of metastasis and relapse in lung carcinoma.
