摘要
目的探讨不同治疗方法对寻常型银屑病表皮细胞凋亡的影响。方法 60例寻常型银屑病患者,随机分成2组,每组30例,分别应用中药银屑病方联合窄谱中波紫外线(narrow band ultraviolet B,NB-UVB)及阿维A联合NB-UVB治疗,免疫组化链菌素亲生物素-过氧化酶连接法(streptavidin peroxidase con-jugated method,SP)检测2组治疗前后生存素(survivin)的表达,并记录治疗前后银屑病皮损面积和严重程度指数(psoriasis area and severity index,PASI)评分,比较治疗前后survivin阳性细胞数的差值及PASI评分差值,同时比较2组治疗过程中不良反应的发生率。结果 2组治疗后survivin表达及PASI评分均下调,治疗前后survivin阳性细胞数差值及PASI评分差值差异无统计学意义,不良反应发生率中药组显著低于阿维A组。结论在调节寻常型银屑病角质形成细胞凋亡及临床疗效方面两者作用相当,因中药的不良反应比较少,其适应证更加广泛。
Objective To reveal the possible roles of different treatment methods in keratinocytes apoptosis of psoriasis vulgaris.Methods 60 cases of psoriasis vulgaris patients were randomly divided into two groups.Each group contained 30 cases.One group was treated with precompounded prescription of Chinese medicine respectively,another group was treated with Avermectin A.Both measurement was combined with narrow band ultraviolet B（NB-UVB）.Streptavidin peroxidase con-jugated method（SP） of immunohistochemistry were used to detect the expression of survivin in two groups and record psoriasis area and severity index（PASI） score before and after treatment.The difference of survivin positive cells and PASI was compared before and after treatment.The incidence of adverse reaction of two groups during treatment was compared.Results Survivin expression and PASI score was decreased after treated with two methods,and survivin positive cells and PASI score had no obvious difference before and after treatment.Incidence of adverse reaction of traditional Chinese medicine group was significantly lower than that of Avermectin A group.Conclusion In the regulation of keratinocyte apoptosis and clinical effects,precompounded prescription of Chinese medicine has equivalent effect compared with Avermectin A.The indications of traditional Chinese medicine is more extensive because it has less adverse reactions.
