摘要
目的探讨米非司酮与卡孕栓用于无痛人流术的临床效果。方法将2009年3-12月在新疆医科大学第一附属医院计划生育门诊就诊、自愿要求行无痛人流术的未产及剖宫产术后的早孕妇女1 260例,随机分为3组:米非司酮组(450例):术前口服米非司酮2d,总量150mg;卡孕栓组(410例):术前60min阴道放置卡孕栓1mg;对照组(400例):术前不用药。观察3组宫颈软化程度、术中出血量、手术时间、不良反应的发生情况。结果 3组宫颈软化程度、术中出血量、手术时间经比较差异有统计学意义(P<0.05);米非司酮组与卡孕栓组宫颈软化程度、副反应的发生情况比较差异无统计学意义(P>0.05);米非司酮组较卡孕栓组术中出血量少、手术时间短,差异有统计学意义(P<0.05)。结论在无痛人流术中,米非司酮与卡孕栓都能有效软化宫颈,但减少术中出血量和缩短手术时间优于卡孕栓,增加了无痛人流术的安全性,值得临床推广应用。
Objective To study the clinical effect of mifepristone and carboprost methylate suppositories in painless induced abortion.Methods One thousand and twenty sixty cases of painless induced abortion that haven′t delivery through vaginal and pregnancy after cesarean section were randomly divided into three groups： mifepristone group：（n=450）,before operation oral mifepristone 150 mg in two days;carboprost methylate suppositories group（n=410）,sixty-minter before operation placed carboprost methylate suppositories 1 mg into vaginal;control group（n=300）： before operation no drug was used.Then observe the degree of cervical dilator,the amount of bleeding,the length of operation and the side effect in three groups.Results The difference of the degree of cervical dilator,the amount of bleeding,the length of operation were significant in three groups（P0.05）;mifepristone group compared with carboprost methylate suppositories group,the degree of cervical dilator and side effect were no statistically significant（P0.05）,but mifepristone group had less amount bleeding and shortly time of operation compare the carboprost methylate suppositories group,the difference were statistically significant（P0.05）.Conclusion Mifepristone and Carboprost methylate suppositories have same effect at cervical dilator in painless induced abortion,but Mifepristone is priority to Carboprost methylate suppositories at the reducing bleeding and shorten the length of operation and it is worthy of recommending for clinical application.
