摘要
目的:观察腹蛇咬伤后咬伤部位早期三棱针打孔法排毒的治疗效果。方法:根据患者中毒时间、病情严重程度采用配伍设计,选取2011-07我部接诊腹蛇咬伤手患者30例,按病情严重程度分级后再分别分入三棱针打孔组、切开排毒组和对照组。入院后同时给予抗腹蛇毒血清、抗感染及中医中药等治疗。观察终点:患者患肢肿胀消褪时间、伤口部位感染、脓肿形成及坏死等情况、其它严重并发症如心肌酶升高、血小板减少及住院时间。结果:①三棱针打孔组、切开排毒组患肢肿胀消退时间明显小于对照组(P<0.05),但前两组间无差异。②三棱针打孔组皮肤感染、脓肿形成及坏死明显低于其他两组。③其他并发症各组间无显著差异。④三棱针打孔组住院时间明显短于切开排毒组和对照组(均P<0.05)。结论:腹蛇咬伤后咬伤部位早期三棱针打孔法排毒疗法可显著缩短患者患肢肿胀消褪及住院时间,并显著减轻伤口局部感染、脓肿及坏死情况的发生;与传统的伤口切开排毒法相比有明显的优势。
Objective：To study the effect of early triangle needle punching therapy around wound on Agkistro- don bites. Method..30 patients of Agkistrodon bits on hands in July of 2011 were selected and divided into punching group, incision group and control group randomly. All patients were also given special antivenom for Agkistrodo- toxin, antibiotics and traditional Chinese medicine after admission. Swelling fade time, infection, necrosis and ab scess of wounded limbs, other serious complications including elevation of myocardial enzymes, thrombocytopenia and hospitalization time were observed. Result： （1）Swelling fade time of punching group and incision group were shorter than that of control group （P〈0.05）. （2） wound infection, tissue abscess and necrosis in punching group were also less than those of incision and control groups （P〈0.05）. （3）There was no significant difference in other serious complications among the three groups. （4）Hospitalization time of punching group was also less than that of incision and control group（P〈0.05）. Conclusion： Early triangle needle punching around wound after Agkistrodon bites could shorten swelling fade and hospitalization time, decrease wound infection, abscess and necrosis and was superior to traditional bite sits incision.
