摘要
目的对2006~2010年5年间某医院住院病区抗菌药物使用量与痰培养鲍曼不动杆菌耐药变化之间的相关性进行评价。方法根据11种抗菌药物平均每日每百张床位所消耗的用药频度(DDDs/100BD)与同期抗菌药物对鲍曼不动杆菌的耐药率,对抗菌药物使用量与耐药率进行相关性统计分析。结果亚胺培南、美罗培南使用量分别与鲍曼不动杆菌对亚胺培南耐药率显著相关,有统计学意义(r=0.939,P=0.005;r=0.880,P=0.021);亚胺培南、美罗培南使用量与鲍曼不动杆菌对美罗培南耐药率显著相关,亦有统计学意义(r=0.972,P=0.001;r=0.911,P=0.012);莫西沙星使用量与鲍曼不动杆菌对亚胺培南、美罗培南耐药率的相关性有统计学意义(r=0.878,P=0.023;r=0.865,P=0.026);其他组数据无统计学意义。结论某医院抗菌药物使用量与鲍曼不动杆菌耐药水平变化之间存在相关关系,这种发现有助于针对抗菌药物高消耗量以及细菌耐药率高的住院病区优化抗菌药物使用策略。
Objective To investigate whether there is a correlation between the rates of antibacterial consumption and the prevalence of antibacterial resistance in Acinet0bacter baumannii in the hospital. Methods Data on antibacterial consumption (from January 2006 to December 2010) were expressed as defined daily doses per 100 bed-days. Rates of antibacterial resistance in Acinetobacter baumannii were collected, and their correlation was analyzed. Results The consumption of imipenem/cilastatin, meropenem was significantly correlated with imipenem resistance in A baumannii ( r =0. 939, P = 0. 005 ; r = 0. 880, P = 0. 021 ). The consumption of imipenem/cilastatin, meropenem was significantly correlated with meropenem resistance in A baumannii (r = 0. 972, P = 0. 001 ; r = 0. 9I 1, P = 0. 012 ). Furthermore, Consumption of moxifloxacin was significantly correlated with A baumannii resistance to imipenem/cilastatin, meropenem (r = 0. 878, P = 0. 023 ; r = 0. 865, P = 0. 026). There were no significant correlation in other drugs. Conclusion There is a relationship between antibacterial consumption and the prevalence of antibacterial resistance in the hospital. These findings may help to optimize antibacterial prescription policies in the hospital, especially in departments that have both high rates of antibacterial consumption and a high prevalence of antibacterial resistance.
