摘要
目的探讨肝硬化(LC)肠道细菌移位(BT)与多器官组织病理及功能变化的相关性。方法 40只雄性SD大鼠,鼠龄3个月左右,体质量(250±20)g。随机分为对照组(NC组,10只)、肝硬化模型组(LC组,30只)。LC组采用改造的复合病因诱导大鼠LC模型,将肠系膜淋巴结匀浆进行细菌培养,以细菌培养结果分为BT阳性组和BT阴性组;留取股动脉血检测肝、肾功能及血氨;行肝、回肠、肾、脑组织病理学检测(苏木精-伊红染色)。结果成功复制了LC大鼠模型。NC组大鼠未发生BT;LC组大鼠细菌移位,阳性率为69.23%,其中大部分为大肠埃希氏细菌(88.9%)。BT阳性组血清白蛋白及血氨水平分别显著低于和高于BT阴性组(均P<0.05)。与BT阴性组比较,BT阳性组有更为显著的肝、脑及回肠组织病理改变。结论BT是LC常见的病理生理变化,其发生与肠道屏障功能减弱、明显的肝功能减弱、潜在增加的发生肝性脑病的可能性相关。
Objective To investigate the effects of bacterial translocation(BT) on vital organs in liver cirrhosis(LC), and determine the relationship between BT and vital organs. Methods Forty male SD rats, aged 3-months old, body weight (250± 20) g, which were randomly divided into 2 groups: control group(NC, n = 10) and LC group(n = 30). LC group was rat cirrhosis model induced by composite etiology pathogenic factors, and the mesenteric lymph node homogenates was cultured for bacterial amplification. The animals were subsequently divided into BT positive group(BT+) and BT negative group(BT-) accordances with the bacterial culture results. Blood specimens were collected from femoral artery for the detection of hepatic function, renal function and ammonia(AMM). Tissue sections of liver, ileum, kidney, and brain were prepared for histological examination (HE staining). Results The LC model was successfully induced. The bacterial translocation was not observed in NC group, whereas the translocation rate in LC group was 69.23 %, most of which were E.coli bacteria(88.9 %). Compared with the BT+ group, the BT- group showed significantly increased serum level of albumin and decreased serum level of AMM(P 〈 0.05). The pathological changes in liver, ileum and brain tissue of the BT+ group were worse than that in the BT- group(P 〈 0.05). Conclusion It is demonstrated that BT is a common pathophysiological change in LC, it could influence liver function and may increase the possibility of occurrence of hepatic encephalopathy.
