摘要
目的:探讨心电图下壁导联的R波切迹诊断房间隔缺损(ASD)的价值。方法:分析19例ASD心电图(ASD组),19例正常人作为对照组(正常对照组)的心电图资料。ASD的诊断均经超声心动图证实,并未行手术矫正。结果:19例ASD心电图中,下壁导联出现R波切迹者18例(94.7%);正常对照组有3例(15.8%)出现R波切迹,ASD组心电图下壁导联出现R波切迹的比率显著高于正常人组(94.7%vs15.8%,P<0.0001)。以心电图下壁导联出现R波切迹诊断ASD的敏感性,特异性,准确性分别为94.7%和88.9%,89.5%。结论:心电图下壁导联R波切迹是诊断ASD的一项有价值的,新的指标。
Objective：To investigate the value of notch on R wave in inferior lead of Electrocardiogram（ECG） diagnosing patients with atrial septal defect.Methods：The data of ECG in 19 patients diagnosed as atrial septal defect（ASD,ASD group） and ECG of 19 normal control subjects（normal control group）were reviewed.All of ASD patients received echocardiography examine and had not been corrected.The data of notch on R wave in inferior lead of ECG for two groups were analyzed and compared.Results：The notch on R wave in lead of inferior leads were observed in 18 cases of 19 ASD patients（94.7%）.The notch on R wave in lead of inferior leads were observed in 3 cases of 19 normal persons（15.8%）.The incidence of notch on R wave in inferior lead of ECG in ASD group was significantly higher than that of normal control group（94.7% vs.15.8%,P0.001）.The sensitivity,specificity and accuracy of this ECG pattern（notch on R wave in aVF lead of ECG）diagnosing ASD were 94.7%,88.9% and 89.5% respectively.Conclusion：The notch on R wave in aVF lead of ECG is a valuable,new index diagnosing atrial septal defect.
