摘要
目的:观察大医院急诊科室从业医护群体职业应激表现及对血糖、血脂影响。方法:收集和分析2011年沈阳军区总医院和解放军第四六三医院医护人员体检资料,选择42例急诊科室医护人员(急诊科室组)和73例门诊科室人员(门诊科室组)为研究对象,进行职业紧张调查表[OSI-R,包括职业任务问卷(ORQ),和个体紧张反应问卷(PSQ)]评估,分析血糖、血脂等项体检结果。结果:与门诊科室组比较,急诊科室组ORQ各因子分及问卷总分[(129.68±31.75)分比(140.37±34.62)分]、PSQ各因子分及总分[(83.29±17.60)分比(96.32±21.75)分]均明显升高(P<0.01～<0.05);急诊科室组空腹血糖[(4.51±0.47)mmol/L比(4.89±0.62)mmol/L]、餐后2h血糖[(5.35±0.74)mmol/L比(5.96±0.90)mmol/L]和总胆固醇[(4.43±0.56)mmol/L比(4.92±0.71)mmol/L]水平亦明显升高,而高密度脂蛋白胆固醇[(1.59±0.27)mmol/L比(1.06±0.19)mmol/L]水平则显著降低(P均<0.01～<0.05)。结论:大医院急诊科室医护人员群体职业应激程度较大,血糖和血脂水平升高。
Objective：To analyze occupational stress response in medical staff from emergency departments of large hospitals and its effect on blood glucose and blood lipids.Methods：Health examination data of medical staff from 463th hospital of Chinese PLA and general hospital of Shenyang military command in 2011 were collected and analyzed.A total of 42 medical staff from emergency department（emergency department group） and 73 medical staff from outpatient department（outpatient department group） were enrolled.All subjects underwent measurements of occupation stress inventory revised edition,blood glucose and blood lipid.Results：Compared with outpatient department group,there were significant increase in factor scores and total score of ORQ [（129.68±31.75） scores vs.（140.37±34.62） scores],PSQ [（83.29±17.60） scores vs.（96.32±21.75） scores]（P0.01～0.05）;there were significant increase in levels of fasting blood glucose [（4.51±0.47） mmol L vs.（4.89±0.62） mmol L],2h postprandial blood glucose [（5.35±0.74） mmol L vs.（5.96±0.90） mmol L]and total cholesterol [（4.43±0.56） mmol L vs.（4.92±0.71） mmol L],and significant decrease in level of high density lipoprotein cholesterol [（1.59±0.27） mmol L vs.（1.06±0.19） mmol L],P0.01～0.05 all in emergency department group.Conclusion：Levels of occupational stress and blood glucose and blood lipids increase among medical staff from emergency departments of large hospitals.
