摘要
目的比较陈旧性FanelliⅢ型膝关节后外侧结构损伤的两种重建方法,探讨其临床结果。方法 2007年10月~2008年9月,46例FanelliⅢ型PCL合并PLC损伤患者,分别采用自体肌腱及同种异体肌腱重建PLC的手术方法。根据IKDC、Lysholm评分标准进行膝关节功能评定。结果随访12~30个月,平均21.38±6.26个月。两组膝关节功能评分﹙Lysholm、IKDC﹚、膝关节活动功能评分均较术前明显改善,差异有统计学意义。后抽屉试验阴性38膝,后抽屉试验阳性1+不稳定6膝,2+不稳定2膝,膝关节后侧稳定性有明显改善。膝关节伸直位、屈曲30°位内翻试验均为阴性。膝关节屈曲30°位外旋试验双侧差均〈10°。结论采用自体肌腱及同种异体肌腱重建PLC,两种方法均能有效地重建PLC,有效恢复膝关节后侧及后外侧旋转稳定性,术后膝关节功能恢复良好。
Objective To compare the two methods of reconstruct the old Fanelli Ⅲ type of posterolateral comer injury of knee, and to evaluate their clinical outcomes. Methods 56 cases of old FanelliⅢ type of combined PCL and PLC injury were treated with autologous tendons and allograft. The patients were followed up for more than one year and evalu- ated according to IKDC, Lysholm scales. Result All of them were followed up for 12 to 30 months with an average of 21.38±6.26 months. Two sets of knee function score ( Lysholm, IKDC ) knee function score improved significantly compared with the preoperative. 38 knee drawer test was negative, 6 knee posterior drawer test was instability positive 1+, 2 knee instability 2% Posterior stabilities were recovered significantly, varus stress test was negative at 30 degree of flexion and full extension, less than 10 degree of external rotation compared with the intact knee in all of the patients. Conclusion Both autologous tendon and allograft tendon can effectively reconstruct PLC, effectively reconstruct the res- toration stability of posterior and posterolateral of knee, knee function have a good recovery.
