北京石景山区老年人群牙周炎与代谢综合征的相关性探讨 被引量:12

摘要 目的:调查北京石景山区老年人群牙周炎和代谢综合征状况,探讨牙周炎与代谢综合征的关系。方法:在2005年对北京石景山区社区的中老年人群进行问卷调查、牙周临床指标及血生化检查,903名个体纳入最终分析。根据牙周炎的严重程度分为无及轻度牙周炎组、中重度牙周炎组,比较中心性肥胖、高血糖、高甘油三酯、高血压和低的高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)等指标以及代谢综合征的比例在两组中的差异;以Logistic回归分析牙周炎与代谢综合征及各组分的相关性。结果:中重度牙周炎组的代谢综合征、高血糖和低HDL-C的比例显著高于无及轻度牙周炎组。调整年龄、性别、吸烟等混杂因素后,中重度牙周炎者患代谢综合征、高血糖和低HDL-C的风险分别是无及轻度牙周炎者的1.524倍、1.527倍和2.349倍。结论:北京石景山区老年人群中,中重度牙周炎者的代谢综合征、高血糖和低HDL-C的比例较高,牙周炎严重程度与代谢综合征、高血糖和低HDL-C有一定的相关性。 Objective: To survey the metabolic status of the middle-aged and aged population with periodontitis in Shijingshan community of Beijing, and investigate the relationship between periodontitis and metabolic syndrome (MS). Methods: The middle-aged and aged population in the community were investigated by questionnaires, periodontal clinical examinations and blood biochemical tests in 2005. A total of 903 subjects were enrolled, who were divided into two groups by severity of periodontitis. Their waist circumferences, values of high-density lipoprotein cholesterol (HDL-C), systolic blood pressure, fasting glucose, MS and its individual components (central obesity, insulin resistance, hypertriglyceridemia, low HDL-C and hypertension) were compared between the two groups. The Logistic regression model was set to analyze the relationship between periodontitis and MS. Results: There was a significantly higher mean of systolic blood pressure, fasting glucose in the subjects with moderate-severe periodonti- tis than that with no-mild periodontitis. With severity of periodontitis increasing, the prevalence of MS, high blood glucose and low HDL-C increased significantly. After adjustment for gender, age, and smoking, the subjects with moderate-severe periodontitis were 1. 524, 1. 527 and 2. 349 times more likely to suffer from MS, high blood glucose and low HDL-C than those with no-mild periodontitis, respectively. Conclusion : With severity of periodontitis increasing, the prevalence of MS, high blood glucose and low HDL-C increased significantly in the middle-aged and aged population of the community in Beijing. Severity of periodontitis is associated with MS, high blood glucose and low HDL-C.
