摘要
目的:对几种袋内冲洗液在重度牙周炎非手术治疗中的临床疗效进行比较和评价。方法:28例重度牙周炎患者龈下刮知/根面平整术后,分别用0.1%碘氧液、3%双氧水、0.2%洗必泰及生理盐水进行牙周袋内冲洗,术后每周冲洗1次,术前、术后28d分别检测MPI、GBI和PD。结果:术后28d,碘氧液、双氧水、洗必泰、生理盐水及对照组MPI发别降低0.83,0.84,0.81,0.79和0.76,各组差别无显著性(P>0.05);GBI降低0.62,0.66,0.62,0.51和0.41,碘氧液、双氧水、洗必泰组高于生理盐水及对照组,差异有显著性(P<0.05),但碘氧液、双氧水、洗必泰组间无显著性差异(P>0.05),生理盐水组与对照组之间亦无显著性差异(P>0.05)。PD分别降低1.90,2.11,2.03,1.91,1.90mm,差异无显著性(P>0.05)。结论:含抗菌成分的袋内冲洗液有助于改善牙周组织的炎症状态,可提高非手术治疗在重度牙周炎中的疗效。
Objective: To compare the effects of subgingival irrigation with different medications after subgingival scaling/root planning (SS/RP) on clinical parameters in advanced periodontitis. Methods: 28 advanced periodontitis patients with at least 5 teeth with probing depth more than 6 mm completed the study. 4 teeth were irrigated with 0.1% iodine oxide (KI+H_2O_2, IO), 3% hydrogen peroxide (HP), 0.2% chlorhexidine (CHX) and 0.9% sodium chloride (SC) respectively, the left one as control. Probing depth (PD), supragingival plaque (MPI), gingival bleeding on probing (GBI) was measured before SS/RP as the baseline and four weeks after the treatment. Results: In the 4th week after SS/RP in combination with irrigation, MPI reduction was 0.76~0.84,without significant difference among these groups(P<0.05).GBI reduction was 0.41-0.66, and that in 0.1% iodine oxide, 3% hydrogen peroxide, 0.2% chlorhexidine was significantly higher than that in 0.9% Sodium chloride irrigation group and in controls(P>0.05). No significant difference was found among 0.1% iodine oxide, 3%hydrogen peroxide and 0.2% chlorhexidine groups(P>0.05), and between 0.9% sodium chloride irrigation group and controls(P>0.05). PD reduction during the treatment was observed without significant differences among the groups. Conclusion: Subgingival irrigation with iodine oxide, 3% hydrogen peroxide, and 0.2% chlorhexidine,combined with root planning can help improve the gingival inflammation status in treating advanced periodontitis.
